The PRIME designation recognizes medicines with potential to address significant unmet medical needs and facilitates enhanced regulatory dialogue for accelerated development and review in the European Union.
NRTX-1001 is currently being evaluated in two ongoing multicenter, open-label Phase 1/2 clinical trials in adults with drug-resistant mesial temporal lobe epilepsy (search) (MTLE (search)), the most common type of focal epilepsy in adults. The company is also preparing to initiate screening for its previously announced pivotal Phase 3 EPIC (EPIlepsy Cell therapy) randomized controlled trial.
Addressing Treatment Limitations
For focal epilepsy patients whose disease does not adequately respond to medications, current treatment options are limited to highly invasive surgical procedures, including lobectomy resection or ablation surgeries, which are brain tissue-destructive. These destructive surgeries carry significant risk of irreversible neurocognitive impairment, with risk levels elevated enough to exclude many drug-resistant patients from eligibility.
NRTX-1001 represents an investigative cell therapy alternative administered through a one-time, minimally-invasive delivery procedure. By integrating into dysregulated neural circuits, secreting GABA, and restoring inhibitory tone from a single dose, NRTX-1001 has the potential to substantially and durably reduce or eliminate seizures in focal epilepsy patients whose disease is inadequately controlled by traditional anti-seizure medications. Due to its non-destructive design, NRTX-1001 may avoid the risk of neurocognitive impairment associated with invasive surgical procedures.
Clinical Trial Results
Neurona presented updated data from its unilateral MTLE (search) trial at the AAN Annual Meeting in April 2025. The results demonstrated a 92% median reduction from baseline in disabling seizures during the 7-12-month efficacy endpoint period and an 80% (4/5) responder rate with greater than 75% seizure reduction at the 12-month endpoint. Durable seizure control was maintained in all (4/4) responders followed for 18-24 months after a single low-dose of NRTX-1001 in Cohort 1.
All subjects (5/5) in Cohort 1 showed significantly improved quality-of-life test scores, with no subject experiencing persistent decline in cognition to date. NRTX-1001 continues to be well-tolerated with no serious adverse events attributed to the cell therapy.
Expanded Clinical Development
The company has expanded its investigation to include patients with MTLE (search) without mesial temporal sclerosis (search) (MTS (search)). The first adult subject with drug-resistant unilateral MTLE without MTS was treated in the ongoing Phase 1/2 clinical trial, with the procedure performed by Dr. Viktoras Palys at the University of Arkansas for Medical Sciences. This expansion cohort will enroll up to ten adult subjects receiving NRTX-1001 via the same one-time image-guided intracerebral procedure used in other study cohorts.
MTLE (search) without MTS (search) represents a notable subset of the drug-refractory MTLE population, characterized by seizure semiology, scalp EEG, and other techniques including magnetoencephalography, intracranial EEG, and PET imaging to localize seizure onset, rather than the typical MRI-detectable hippocampal scarring seen in MTS cases.
Regulatory Pathway and Future Plans
The EMA awarded PRIME designation based on robust preclinical evidence, emerging clinical data, and the unmet need in patients with drug-resistant focal epilepsy (search). This designation provides Neurona with eligibility for early scientific advice, protocol assistance, accelerated assessment, and other regulatory incentives in Europe. Under European Union regulations, the designation also simplifies NRTX-1001's regulatory pathway by allowing for a single centralized Marketing Authorization Application submission.
Neurona is also enrolling a second Phase 1/2 trial for adult subjects with drug-resistant bilateral MTLE (search), both with and without MTS (search). Following discussions with FDA under the Regenerative Medicine Advanced Therapy (RMAT) designation granted in June 2024, the company has begun preparations for its prospective Phase 3 EPIC trial, expected to initiate in the second half of 2025. Results from the Phase 1/2 and Phase 3 trials in both unilateral and bilateral MTLE populations are intended to support a prospective marketing application for treating drug-resistant MTLE.
The company plans to present updated data from across the clinical development program for NRTX-1001 in MTLE (search) at the Annual Meeting of the American Epilepsy Society in December 2025, including early data from recently added cohorts of bilateral and non-MTS (search) MTLE subjects.
