NICE Approves Keytruda as First Perioperative Immunotherapy for Head and Neck Cancer in 20 Years

MSD's Keytruda (pembrolizumab) has received NICE approval as the first perioperative immunotherapy for locally advanced head and neck squamous cell carcinoma (LA HNSCC), ending a two-decade drought in treatment advances for this challenging cancer type. The recommendation covers patients with resectable disease whose tumors express PD-L1 (search) with a Combined Positive Score (CPS) of 1 or more.
Treatment Protocol and Clinical Evidence
The approved regimen involves administering pembrolizumab as neoadjuvant therapy prior to surgery, followed by adjuvant treatment in combination with standard-of-care radiotherapy, with or without concomitant cisplatin, and then continued as monotherapy. Specifically, patients receive two cycles of the PD-1 (search) inhibitor before surgery and 15 cycles after surgery, added to standard care including post-surgery radiotherapy and, in some cases, chemotherapy.
The approval is supported by robust data from the Phase III KEYNOTE-689 trial, which demonstrated a statistically significant improvement in event-free survival (EFS), the study's primary endpoint, versus adjuvant radiotherapy alone. In the CPS ≥1 population, the pembrolizumab-based regimen reduced the risk of disease recurrence, progression or death by 30%. Most notably, median EFS was doubled from 29.6 months in the comparator arm to 59.7 months in the pembrolizumab arm.
Addressing Significant Unmet Medical Need
Head and neck squamous cell carcinoma accounts for approximately 90% of all head and neck cancers, which represent the ninth most common cancer type in the UK. With around 12,800 new cases and 4,700 deaths annually, and mortality rates projected to rise by 12% by 2038-40, the unmet need in this indication is considerable. Around two-thirds of patients are diagnosed when they have locally advanced disease.
Standard treatment had remained largely unchanged since the early 2000s, limited to surgery followed by post-operative radiotherapy with or without chemotherapy, with persistently high recurrence rates and poor long-term outcomes. Treatment options for LA HNSCC can be effective but put patients at risk of impairing their ability to speak, swallow, breathe, see, smell and taste, as well as their mental health and personal appearance.
Safety Profile and Quality of Life
Participant-reported outcomes from KEYNOTE-689 indicated stable health-related quality of life during neoadjuvant pembrolizumab therapy, with no overall detrimental impact on disease-related symptom scores versus the control arm. The safety profile was broadly comparable between groups for most adverse events, though immune-mediated adverse events of any grade were more frequent in the pembrolizumab group (43.2% vs 10.2%).
Clinical and Economic Value
NICE concluded that "there is enough evidence to show that pembrolizumab provides benefits and value for money, so it can be used routinely across the NHS in this population." The guidance noted that "clinical trial evidence shows that neoadjuvant and adjuvant pembrolizumab increases how long people have before their condition gets worse. The evidence also suggests that it may increase how long people live compared with usual treatment."
Implementation and Patient Impact
The final draft guidance sets a 90-day implementation period for NHS funding of Keytruda in this setting. Pembrolizumab is already approved in the UK for metastatic or unresectable recurrent HNSCC since 2020. This NICE recommendation extends its use earlier in the treatment pathway, offering a new option for patients with surgically resectable disease and expanding the role of PD-1 (search) inhibition across the HNSCC continuum of care.

For the head and neck cancer community, this brings real hope, not just in treating the cancer but in the possibility that fewer people may face the most severe long-term effects after treatment."
