NICE Expands Access to AstraZeneca's Lokelma as Hyperkalemia Treatment Threshold Lowered

This represents a significant reduction from the previous threshold of 6.0 mmol/L or higher, enabling earlier intervention in hyperkalemia management.
Professor Jim Burton, a consultant nephrologist at University Hospitals of Leicester, explained the clinical significance of this change: "Because hyperkalemia (search) at a level of 6.0 mmol/L represents such an immediate health concern for so many healthcare practitioners and prescribers, it is highly likely that patients will have had their foundation treatments for heart failure (search) and kidney disease stopped or reduced in dose before reaching that point."
The lower threshold enables proactive management of hyperkalemia (search) at an earlier stage, potentially allowing patients to remain on guideline-recommended RAAS inhibitor therapies that protect heart and kidney function.
Cost-Effectiveness Assessment Under New Framework
Lokelma has become one of the first medicines evaluated under NICE's recently raised cost-effectiveness threshold, which increased from £20,000 to £30,000 per quality-adjusted life year (QALY) to £25,000 to £35,000. This adjustment, implemented at the start of this month, recognizes that the decades-old threshold had become misaligned with the value of modern medicines.
The decision was supported by real-world evidence demonstrating Lokelma's clinical effectiveness and its ability to help patients maintain critical cardiovascular and renal protective therapies.
Clinical Impact and Market Performance
Hyperkalemia (search) is generally associated with the use of RAAS inhibitor therapies for treating chronic kidney disease (search) and heart failure (search), and significantly raises the risk of serious complications including hospitalization, major cardiovascular events, and death. The condition creates a treatment dilemma where life-saving medications may need to be reduced or discontinued due to elevated potassium levels.
Tom Keith-Roach, president of AstraZeneca UK, noted that real-world evidence data shows "a clear relationship between potassium levels and clinical outcomes and demonstrates lower rates of reducing or discontinuing RAAS inhibitors (search) in patients treated with Lokelma."
Lokelma has emerged as a fast-growing mid-tier product for AstraZeneca, with sales rising 28% to $698 million last year, outpacing its main competitor CSL (search)'s Veltassa (patiromer sorbitex calcium). The company has projected that Lokelma could eventually cross the $1 billion blockbuster threshold.
Mechanism and Clinical Differentiation
Unlike existing organic polymer-based treatments, Lokelma is an inorganic crystalline potassium binder that selectively captures potassium throughout the gastrointestinal tract. In vitro studies have confirmed that its potassium selectivity is more than 125 times higher than existing treatments, with its non-absorbable nature cited as a key differentiator.
Clinical trial data has demonstrated Lokelma's rapid onset of action, with potassium reduction occurring within one hour of administration. In the Phase 3 ZS-003 study involving 753 patients with hyperkalemia (search), 86.4% of patients reached normal potassium ranges within 48 hours compared to 47.8% in the placebo group.
The HARMONIZE study showed mean serum potassium levels decreased from 5.6 mmol/L to 4.5 mmol/L within 48 hours, with patients maintaining stable, low potassium levels during the maintenance phase. Long-term data from the ZS-005 study indicated that 88% of patients maintained normal potassium levels after up to 12 months of treatment, while 87% of patients previously using RAAS inhibitors (search) were able to continue or increase their treatment.
