Novel Resistance Markers Discovered
The research team, led by Dr. Ateequllah Hayat, Lecturer in Drug Development at City St George's, University of London (search), discovered nine genetic markers consistently altered in lapatinib-resistant breast cancer (search) cells. Remarkably, seven of these markers—HPGD (search), FASN (search), TPM1 (search), CALD1 (search), PCP4 (search), AKR7A3 (search), and KRT81 (search)—had never been linked to HER2-positive breast cancer (search) or lapatinib resistance before. The remaining two markers, EGFR (search) and SCIN (search), were already known to be involved in resistance mechanisms.
"Over 70% of breast cancer (search) patients relapse within 5 years of treatment, and drug resistance remains one of the biggest challenges in cancer care," Hayat explained. "By combining several advanced techniques, we were able to uncover subtle but critical changes in the cancer cells that were previously invisible."
Paradoxical DNA Packaging Behavior
Using a sophisticated multi-layered approach, the researchers examined lapatinib-resistant HER2 (search) cancer cells at multiple molecular levels. They analyzed chromatin accessibility, gene expression patterns, and protein production to create three "molecular maps" that revealed the most consistent changes driving drug resistance.
The study uncovered a paradoxical behavior in lapatinib-resistant cells. While their DNA was more tightly packaged overall with reduced chromatin accessibility compared to normal HER2 (search) cancer cells, specific regions near key resistance genes were more open and active.
"What we found was quite interesting. In this model, the opening up of the DNA chromatin is associated with oncogenic activation, but rather than the classical opening up associated with oncogenic activation, we instead saw that lapatinib-resistant cells displayed globally reduced chromatin accessibility," Hayat explained. "That means that the DNA within these lapatinib-resistant cells actually became more compacted."
Epigenetic Modifications Drive Resistance
RNA sequencing revealed at least 17 differentially expressed histone-modifying enzymes, all playing key roles in epigenetic regulation. This evidence suggests that lapatinib was not only forcing cancer cells to develop resistance but also mutating their DNA to evade the therapy's effects overall.
The identified genetic markers were found to increase stress responses, actin remodeling, and metabolic reprogramming—all mechanisms that contribute to drug resistance. Additionally, lapatinib-resistant HER2 (search) cells exhibited morphological changes, becoming more irregular and less spherical with protrusions that could facilitate aggressive invasion of healthy cells.
Cross-Cancer Implications
The research team validated their findings by examining lung cancer (search) cells, discovering that two resistance-associated genes—FASN (search) and HPGD (search)—were also elevated with lapatinib resistance in this different cancer type. This finding suggests the nine-marker resistance signature may extend beyond breast cancer (search).
"Our results suggest that our nine-marker resistance signature may be found in other types of cancer beyond breast cancer (search)," Hayat noted. "This work opens the door to developing biomarker-guided therapies that could prevent or reverse drug resistance."
Clinical Translation Pathway
The next phase involves validating these findings in additional cancer models and patient samples to confirm the genetic signatures' role in stimulating drug resistance. Hayat anticipates significant development hurdles, noting this remains at the discovery stage with multiple validation steps required.
"If we validate this signature in other cancer models and in patient samples and really get to grips with the fact that this is truly involved in drug resistance, we could develop liquid biopsy-based assets," Hayat said. "Detecting these changes in expression or epigenetically linked signatures from circulating tumor DNA or RNA could signal emerging resistance before clinical progression."
The diagnostic tools still requiring development include liquid biopsies or tissue-based panels that could help clinicians easily detect patients at risk for drug resistance. Rather than waiting for resistance to develop, physicians could potentially stratify patients based on molecular signatures and adjust therapy proactively.
"We have developed a biological system to understand drug resistance, and we were also able to validate and confirm some of these biomarkers in a completely different system," Hayat explained. "Rather than waiting for these patients to become resistant, clinicians could potentially stratify patients based on the molecular signature or molecular risk and adjust the therapy proactively."
The research represents a significant step toward personalized cancer treatments, with the ultimate goal of making resistance a predictable and preventable process rather than an inevitable outcome.
