Novel Pseudomonas aeruginosa Strains Emerge as Significant Cause of UTIs in Gulf Coast Region

A new study from the University of South AlabamaView company profile has revealed that Pseudomonas aeruginosa (search)Search disease is emerging as a more significant cause of urinary tract infections (UTIs) along the Gulf Coast than previously recognized, accounting for approximately 4% of local UTI cases — nearly double the national average.

The research, published in the American Society for Microbiology's journal mSphere, was led by Allyson Shea, Ph.D., an assistant professor of microbiology and immunology at the Frederick P. Whiddon College of Medicine, with Rachel Fleck, an incoming first-year medical student, serving as first author.

"Pseudomonas aeruginosa (search)Search disease is relatively understudied in the UTI field compared to pathogens like E. coli," said Shea. "Most of what we know about Pseudomonas virulence comes from lung and wound infection models, but the urinary tract is a very different environment. Our goal was to better define the genomic and phenotypic characteristics of urinary isolates specifically."

Genomic Analysis Reveals Uncharacterized Diversity

The research team analyzed 55 bacterial samples collected from patients with UTIs, examining genetic differences, pathogenic mechanisms, and antibiotic response profiles. The genetic analysis uncovered numerous previously unidentified strains circulating in the local patient population.

"It suggests there is still substantial uncharacterized diversity among urinary P. aeruginosa isolates," Shea noted. "Many of the strains circulating in our local patient population did not match previously described sequence types, which highlights how little we still know about P. aeruginosa in the urinary tract."

The study also identified associations between specific bacterial strains and particular patient populations. One strain appeared more frequently in patients with urinary catheters, while another was associated with diabetes, suggesting potential links between underlying clinical factors and strain-specific infections.

Antibiotic Resistance Patterns Raise Concern

Antibiotic resistance emerged as a significant concern in the findings. Nearly one-third of the bacterial isolates demonstrated resistance to levofloxacinView drug details, a commonly prescribed fluoroquinolone antibiotic. Furthermore, several strains exhibited multidrug resistance, making infections substantially more difficult to treat with standard therapeutic regimens.

These resistance patterns underscore the clinical challenge posed by P. aeruginosa UTIs, particularly given that the pathogen has not historically been a focus of urinary tract infection research.

Regional Prevalence and Future Directions

While the study documented a higher regional prevalence of P. aeruginosa UTIs in the Gulf Coast healthcare system compared to published national estimates, the underlying reasons remain unclear.

"Our study identified a higher prevalence of P. aeruginosa UTIs in our regional healthcare system compared to published national estimates, but we do not yet know why," Shea said. "It is likely multifactorial. Factors such as climate, patient population, healthcare-associated exposures and underlying comorbidities may all contribute, but larger epidemiologic studies would be needed to determine that."

The researchers emphasized that the findings do not carry immediate implications for patient care. However, the work establishes an important foundation for future investigation into how these infections develop and persist in the urinary tract.

"There are no immediate implications for patient care from this study, but we hope this work helps highlight the genomic and phenotypic diversity of P. aeruginosa strains causing UTIs," Shea concluded. "Defining that diversity is an important first step toward understanding how these infections develop and persist in the urinary tract."
