Novo Nordisk's CagriSema Outperforms Ozempic in Diabetes Trial, Company Plans Pediatric Obesity Study

Novo Nordisk's investigational dual-acting drug CagriSema has demonstrated superior efficacy compared to Ozempic in treating type 2 diabetes (search), while the company simultaneously advances plans for pediatric obesity (search) trials, signaling continued confidence in the combination therapy despite earlier market disappointments.
Superior Diabetes Control in Head-to-Head Trial
In the REIMAGINE 2 study, CagriSema outperformed Ozempic in both blood glucose control and weight management over 68 weeks of treatment. The highest dose of CagriSema reduced hemoglobin A1c levels by an average of 1.91 percentage points from a baseline of 8.2%, compared to a 1.76% reduction with Ozempic—a statistically significant difference. The placebo group experienced a 0.09% increase in HbA1c levels.
Weight loss results were equally impressive, with CagriSema achieving an average 14.2% weight reduction compared to 10.2% with Ozempic and 1.5% with placebo. The dual-acting drug's performance was significantly better than Ozempic in this measure as well.
"By combining semaglutide and cagrilintide, we're seeing superior outcomes in both blood glucose control and weight reduction beyond those achieved with each therapy individually," said Martin Holst Lange, Novo Nordisk's head of R&D.
Expanding into Pediatric Population
Despite earlier setbacks in adult obesity (search) trials, Novo Nordisk is moving forward with a clinical study investigating CagriSema in children and adolescents. The study, set to begin in January, will enroll 460 participants as young as eight years old across 97 test centers in China, Europe, and the United States.
This pediatric expansion comes after the company's REDEFINE 2 study in March disappointed investors and analysts. That phase 3 trial in 1,206 overweight adults with type 2 diabetes (search) showed 15.7% weight loss after 68 weeks compared to 3.1% with placebo—results that failed to demonstrate superiority over Eli Lilly's obesity (search) drug Zepbound and fell short of market expectations for 20% weight loss.
Regulatory Strategy and Market Positioning
CagriSema, a once-weekly injectable combining semaglutide (the active ingredient in Ozempic and Wegovy) with amylin (search) agonist cagrilintide, has already been submitted for obesity (search) approval in December. The filing was supported by REDEFINE 1 and 2 studies, which demonstrated that 40% of patients achieved at least 25% body weight reduction, while 23% lost 30% or more.
Novo Nordisk is now considering additional regulatory filings for diabetes treatment based on the REIMAGINE trial results. The company plans to discuss with regulators using data from REIMAGINE 1, REIMAGINE 2, and the ongoing REDEFINE 3 cardiovascular outcomes study as the basis for diabetes marketing applications.
"The results strengthen our belief that CagriSema could be the first amylin (search)-based combination therapy and a promising treatment option for individuals with type 2 diabetes (search)," Lange added.
Market Implications
Despite earlier disappointments, some analysts predict CagriSema could generate $7 billion or more in annual peak sales, making it a valuable addition to Novo Nordisk's incretin medicine portfolio. The drug represents a key component in the company's strategy to compete against Eli Lilly's increasingly potent challenge, particularly the dual GIP (search)/GLP-1 (search) agonist tirzepatide, marketed as Mounjaro for diabetes and Zepbound for obesity (search).
Lange expressed satisfaction with the latest results, calling them "a confirmation of the very strong weight loss data seen with CagriSema in the obesity (search) trials," suggesting the company maintains confidence in the drug's therapeutic potential across multiple indications.
