Omadacycline Shows Promise in Phase II Trial for Nontuberculous Mycobacterial Pulmonary Disease

The exploratory Phase II trial (NCT04922554) enrolled 66 patients aged 18 years or older diagnosed with NTM-PD (搜索) caused by MABc according to Infectious Diseases Society of America/American Thoracic Society guidelines. Patients were randomized to receive either 300mg of oral omadacycline once daily for 84 days or an oral placebo for the same duration.
Significant Bacterial Burden Reduction
The study demonstrated substantial improvements in bacterial burden measures across multiple assessment methods. Using a scoring system based on colony counts on agar plates, omadacycline cultures showed consistent reductions in culture scores at days 28, 56, and 84 compared to placebo cultures. Overall, 76.5% of patients treated with omadacycline experienced a decrease in their culture scores, compared to 45.8% of placebo-treated patients.
Similar results were observed in sputum cultures analyzed in liquid media, with greater reductions in bacterial burden in the omadacycline group. Among omadacycline-treated patients, 73.0% had sputum cultures with growth in liquid medium only, compared to 39.1% of placebo-treated patients, indicating lower bacterial burden.
Most notably, 53.8% of participants in the omadacycline-treated group produced negative sputum cultures at day 84 of treatment, compared to 25.0% of participants in the placebo group. The minimum inhibitory concentrations for omadacycline remained unchanged from baseline to day 84, indicating no development of resistance during the study period.
Addressing Critical Unmet Medical Need
NTM are opportunistic pathogens that cause infections in patients with pre-existing health conditions or compromised immune systems. The most frequent symptoms of NTM-PD (搜索) include cough, fatigue, fever, loss of appetite, shortness of breath, and weight loss. According to GlobalData, prevalent cases of pulmonary NTM are expected to increase by 7.2% in the seven major pharmaceutical markets between 2023 and 2033.
The lack of NTM therapeutics represents one of the biggest unmet needs within the NTM market. Nearly all drugs currently used to treat NTM infections are used off-label, with many being broad-spectrum antibiotics repurposed for NTM treatment.

Paratek's omadacycline, a novel tetracycline derivative, is already marketed in the US and China under the brand name Nuzyra for treating adults with community-acquired bacterial pneumonia (搜索) and acute bacterial skin and skin structure infections (搜索). For the NTM-PD (搜索) indication, omadacycline has received fast-track designation from the US Food and Drug Administration (搜索) (FDA) and orphan drug designation from both the FDA and the European Medicines Agency (搜索).
