Oncology Drug Development in the UK: 25-Year Analysis Reveals Shifting Trial Landscape and Regulatory Reboot

A structured analysis of oncology clinical trial activity across the UK, EU, USA, and China over the past quarter-century has revealed significant shifts in the UK's position within the global cancer (search)Search disease research landscape. The study, published in the British Journal of Cancer, examined interventional cancer trials registered on ClinicalTrials.gov between 1 January 2000 and 31 December 2025, providing the most comprehensive assessment to date of how regulatory changes, geopolitical events, and public health crises have shaped oncology drug development in the UK.

The analysis, conducted by Paulina Czajka-Francuz and an international multidisciplinary team spanning academia, clinical practice, and industry, comes as the UK prepares for a major regulatory reboot with the new UK Clinical Trials Regulation (UK CTR) taking effect on 28 April 2026.

A Quarter-Century of Trial Activity: Peaks, Troughs, and Partial Recovery

The first fifteen years of the century saw an almost steady increase in cancer (search)Search disease clinical trials conducted in the UK, interrupted only by a temporary drop in 2008 attributed to the global financial crisis, which affected both commercial and non-commercial studies. The UK reached its zenith in 2017, with 333 newly initiated oncology trials. However, this was followed by a decline to 230 studies by 2020, a period overlapping with the Brexit referendum and the UK's formal departure from the EU.

"Regulatory changes, a reduction in application numbers, and observed longer approval timelines — with a median of 247 days from regulatory submission to first patient dosing in 2020 — together with potential constraints in resource allocation, may have contributed, among other factors, to the observed decline," the authors note.

A recovery trend emerged from 2020, driven primarily by growth in commercial studies, though this was again disrupted by the COVID-19 (search)Search disease pandemic. Trial numbers fell from 290 in 2021 to 218 in 2023. By the end of 2025, the UK recorded 214 oncology trials — still well below the 2017 peak.

Population-Adjusted Trial Density: A More Nuanced Picture

The study emphasises that raw trial counts alone can be misleading when comparing regions of vastly different population sizes. Across the 2021–2025 interval, population-adjusted trial density placed the EU (28.6 trials per million) and the USA (28.3) at the forefront, with the UK at 18.2 and China at 6.3 trials per million.

A single-year snapshot for 2025 showed the USA with the highest trial density (5.4 per million), followed by the EU (4.9), the UK (3.1), and China (1.4) — despite China's substantial absolute trial volume of 1,944 studies.

"The UK consistently demonstrates a level of trial density that is intermediate between the highest-intensity regions and emerging large-scale research environments such as China, indicating a sustained ability to deliver oncology trials at scale relative to population size," the authors write.

Shifting Sponsorship Dynamics

Commercially funded trials have predominated in the UK across all periods analysed. The increasing proportion of industry-sponsored studies after 2020 aligns with efforts to position the UK as a competitive destination for commercial research. In contrast, non-industry-funded trial numbers have declined, which the authors suggest "may reflect capacity constraints within the NHS and academic sector, particularly following the COVID-19 (search)Search disease pandemic."

Evidence from the pandemic period indicates that trial conduct and recovery differed by sponsor type, with patterns that "appeared to favour commercial studies." The authors caution that this shift "may not necessarily indicate reduced academic engagement but rather highlights a changing funding balance in which commercially sponsored trials may be more resilient to systemic shocks and resource competition."

The 2026 Regulatory Reboot

Since leaving the EU, the UK's legal framework has been based on the EU Clinical Trials Directive (2001/20/EC). The Medicines for Human Use (Clinical Trials) Amendment Regulations 2024 — the new UK CTR — becomes effective on 28 April 2026. Key features include a combined regulatory and ethics review process through the Integrated Research Application System (IRAS), a risk-proportionate approach including a new "notifiable trial" category for low-risk studies, and a target of 150 days from regulatory submission to first patient recruitment, down from approximately 250 days in 2022.

The MHRA has already demonstrated acceleration in initial clinical trial assessment timelines, averaging 29 days in recent reporting cycles. However, the authors note that "the majority of oncology trials do not fall into" the notifiable trial category, as the provisions are not applicable to unlicensed drugs.

China's Meteoric Rise

The study documents China's remarkable trajectory: from 2,251 trials in 2000–2015 to 8,908 in 2021–2025, driven by the 2015 CFDA (now NMPA) regulatory reform, a large patient pool, lower trial costs, and substantial non-commercial investment. China's oncology research has been "historically characterised by a strong contribution from publicly and academically funded research, particularly through investigator-initiated studies conducted within major academic centres."

Restoring Competitiveness

The authors outline several initiatives that may support the UK's long-term competitiveness: the £15.5 billion Life Sciences Investment Package (June 2023), health database investments of up to £600 million, renewed participation in Horizon EU with nearly £500 million in competitive EU grants, and the transition from the UK Clinical Research Network to the Research Delivery Network (search)Search term (RDN) in October 2024.

"The UK needs to leverage more the scientific expertise, regulatory reforms and the healthcare infrastructure to remain competitive," the authors conclude, while noting that the evolving landscape — including renewed interest in cancer (search)Search disease vaccines building on COVID-19 (search)Search disease mRNA technology and increasing numbers of advanced therapy medicinal products — presents both challenges and opportunities for UK oncology research.
