Oral Antibiotics Noninferior to IV Therapy for Severe Pediatric Pneumonia, Landmark PediCAP Trial Shows

Children hospitalized with severe community-acquired pneumonia (search) can safely transition from injectable to oral antibiotics once they begin to recover, allowing earlier hospital discharge and completion of treatment at home, according to results from the landmark PediCAP trial published in The Lancet.
The international clinical trial, led by researchers from City St George's, University of London (search), the UCL Innovative Clinical Trials Unit (search), and partners across Africa and Europe, enrolled 1,101 children aged two months to six years across 13 hospitals in South Africa, Uganda, Zambia, Zimbabwe, and Mozambique. It represents one of the largest studies ever conducted to assess antibiotic treatment strategies for severe childhood pneumonia (search) in Africa.
"Every year millions of children around the world are admitted to hospital with severe pneumonia (search). Our study shows that once a child is clinically improving, it is safe to switch from injectable to oral antibiotics, and complete treatment at home," said Professor Julia Bielicki, Professor of Pediatric Infectious Diseases at City St George's, University of London (search) and co-lead author of the study.
A Trial Designed for Efficiency
The PediCAP trial employed an innovative open-label, parallel group, multi-arm design that simultaneously evaluated different antibiotics and treatment durations. All children began treatment with a WHO-recommended injectable antibiotic. Of the participants, 100 were assigned to receive the recommended five days of IV antibiotic treatment, 502 were assigned to step down to oral amoxicillin after at least 24 hours of IV treatment, and 499 were assigned to step down to combined amoxicillin-clavulanate (search). Children in the step-down groups were further split evenly into five subgroups receiving four, five, six, seven, or eight total days of antibiotics.
Patients in the amoxicillin and co-amoxiclav groups received a median of two days of IV antibiotics before stepping down to oral therapy (IQR, 1.5–3.4 days and 1.5–3.5 days, respectively).
Dr. Michelle Clements, Principal Statistician at UCL Innovative Clinical Trials Unit (search) and co-lead author, explained the significance of the trial design: "PediCAP is the first large-scale study to use our innovative multi-arm trial design to evaluate different antibiotics and treatment durations at the same time. Rather than simply comparing one short course with one longer course, this approach allowed us to establish that the shortest studied treatment strategy was effective and safe, while also helping us to understand the relationship between treatment length and effect."
Comparable Outcomes Across All Groups
At 28-day follow-up, researchers found no significant differences in death or readmission among any of the assigned treatment groups. Rates of hospital readmission or death within 28 days were 6% for oral amoxicillin, 7% for oral amoxicillin-clavulanate (search), and 6% for injectable antibiotics—demonstrating that early switch to oral treatment is a safe and effective strategy.
Notably, standard amoxicillin performed just as well as the broader-spectrum amoxicillin-clavulanate (search), supporting the use of a treatment that is cheaper and more widely available. There was also no evidence that co-amoxiclav was superior to amoxicillin alone.
Shorter Durations Prove Sufficient
The trial also compared outcomes across different total antibiotic course lengths. A total antibiotic course of four to five days was as effective as longer courses of seven or eight days, suggesting many children can be treated successfully with substantially less antibiotic exposure than is often used in practice.
Children who switched to oral antibiotics left hospital approximately one day earlier compared to those who remained on injectable treatment for the full five days.
Implications for Global Practice
Current WHO guidelines recommend five days of injectable antibiotics for children hospitalized with severe community-acquired pneumonia (search), often requiring them to remain hospitalized even after substantial clinical improvement. Longer hospital stays are more expensive, place higher burdens on already pressured healthcare systems, increase the risk of hospital-acquired antibiotic-resistant infections, and impact the wellbeing of children and their families.
"This simple change could help children get back to their families sooner, reduce pressure on busy hospitals, lower healthcare costs and avoid sometimes catastrophic financial impacts on families from lost caregiver earnings," Professor Bielicki said. "Because amoxicillin is affordable and widely available, these findings have the potential to change clinical practice and improve care for children around the world."
Bielicki noted that the team's priority is working with WHO and national guideline groups to incorporate these findings into treatment guidelines for severe pediatric pneumonia (search). Additional research is underway to quantify the cost savings of stepping down to oral antibiotics compared with keeping patients hospitalized for five days of IV treatment. The team is also exploring whether similar approaches could safely reduce antibiotic exposure for other serious childhood infections.
