Palbociclib Triple Therapy Shows Significant Survival Benefit in HER2-Positive Breast Cancer Trial

Trial Design and Patient Population
The multicenter, prospective, open-label study randomized 73 patients with HER2 (search)-positive, hormone receptor-positive, PAM50 luminal A/B advanced breast cancer who had received prior treatment. Patients were randomized 1:1 to receive either the triplet combination of palbociclib, trastuzumab, and letrozole as endocrine therapy, or treatment of physician's choice (TPC).
In the control arm, 48.5% of patients received trastuzumab plus chemotherapy, 39.4% received T-DM1 (Kadcyla), and 12.1% received trastuzumab combined with endocrine therapy. The study's primary endpoint was investigator-assessed progression-free survival evaluated according to RECIST 1.1 criteria.
Efficacy Results
The triplet regimen achieved a statistically significant improvement in progression-free survival compared to standard care, with a stratified hazard ratio of 0.52 (95% CI, 0.29-0.95; p = 0.03). At 24 months, progression-free survival rates were markedly different between treatment arms: 24.0% for the triplet combination versus 4.3% for treatment of physician's choice.
The overall response rate also favored the experimental arm, reaching 18.9% (95% CI, 8.6-35.7) for the triplet regimen compared to 7.1% (95% CI, 1.2-25.0) for standard care.
Safety Profile
Grade 3 or higher adverse events occurred in 61.5% of patients receiving the triplet therapy, with neutropenia being the most frequently reported severe toxicity at 53.9%. Despite the relatively high frequency of severe adverse events, the safety profile was considered acceptable, as no patients required permanent treatment discontinuation due to toxicity.
Clinical Significance
According to study director Aleix Prat from the Clínic Barcelona Comprehensive Cancer Center, these results suggest that combining palbociclib, trastuzumab, and endocrine therapy "could represent a promising strategy to improve outcomes in this specific subset of advanced breast cancer patients."
The findings are particularly relevant given that HER2 (search) alterations are well-established drivers of disease associated with higher risk of recurrence and poor therapeutic response.
