The central question before the jury is not whether she committed the acts—that is undisputed—but whether she was criminally responsible, or whether severe postpartum psychosis (search) had severed her grip on reality.
The case, unfolding as Massachusetts considers legislation to prioritize treatment over punishment for new mothers with postpartum psychiatric illness, has become a flashpoint for examining the fragmented, underfunded postpartum mental health system in the United States.
A psychiatric emergency poorly understood
Postpartum psychosis (search), the condition Clancy's defense team says she experienced, is a psychiatric emergency at the extreme end of the perinatal mood disorder spectrum. Yet public understanding remains dangerously incomplete. "Some narratives within the case and in the general public surrounding this case continue to treat functioning and postpartum psychosis as mutually exclusive: if Clancy could run errands and hold it together in front of others, she must have been fine," notes reporting from Your Local Epidemiologist. In reality, a mother can move from disorganized and incoherent to composed within the same day—arriving at appointments looking put-together while privately losing touch with reality, hearing voices, experiencing delusions or paranoia.
Dr. Lee Cohen, director of the MGH Center for Women's Mental Health in Boston, described the condition to Massachusetts lawmakers: "Postpartum psychosis (search) is brain circuitry disconnected resulting in psychotic symptoms including paranoia, delusional thinking and hallucinations. New moms lose touch with reality, placing themselves and their children at risk." He warned that it could appear quickly and without prior psychiatric diagnoses, while also testifying that postpartum psychosis is "exquisitely treatable with medications and therapy."
Despite its severity, postpartum psychosis (search) has no formal standalone diagnosis. It is classified under other disorders using a "with peripartum onset" specifier, and without a diagnostic standard, no dedicated screening tool exists. In the U.S., 70% of women say they wish they had known more about postpartum mental health before giving birth, with the number even higher among women of color. A U.K. survey found that only 6% of pregnant women receive any information about postpartum psychosis at antenatal classes, despite the condition affecting roughly the same number of families as Down syndrome each year.
A cascade of system failures
Clancy repeatedly sought help: she attended a day program for postpartum depression (search), disclosed thoughts of self-harm, received inpatient care just 19 days before her children's deaths, and had ongoing psychiatric appointments, including one the day before the killings. She was prescribed 13 psychiatric medications by multiple providers across settings—including three benzodiazepines (Klonopin, Valium, and Ativan), mood stabilizers and antidepressants (Zoloft, Remeron, Prozac, trazodone, Lamictal, and Seroquel), as well as the anti-anxiety drug hydroxyzine and the sedative-hypnotic Ambien.
Defense attorney Kevin Reddington called this a "horrific overmedication of drugs that caused homicidal ideation, suicidal ideation," adding: "Our society fails miserably in treating women with postpartum depression (search) or even postpartum psychosis (search). It's medicate, medicate, medicate. Throw the pills at you and then see how it works."
Yet the deeper failure, experts argue, is structural: no single provider was responsible for seeing the whole picture. "Who was responsible for seeing the whole picture? Our fragmented health system means that one doctor rarely knows what another doctor is doing or has done. No information is shared, including prescriptions, so a patient's full picture is often lost," according to the analysis by Your Local Epidemiologist. The entire burden of coordination falls on the patient—a mother expected to recognize and monitor her own psychiatric state while sleep-deprived, still physically healing, and caring for a newborn.
The research deficit
The gaps in clinical infrastructure mirror a chronic underfunding of women's health research. In fiscal year 2023, the NIH categorized approximately $4.6 billion as funding for research on women's health—roughly one-tenth of its nearly $45 billion biomedical research budget. A Nature analysis found that conditions disproportionately affecting women, including migraine, endometriosis, and anxiety disorders, receive far less funding relative to the burden they cause, and female-dominant diseases are underfunded more severely than male-dominant ones.
Broader systemic fractures
Beyond Clancy's individual case, the landscape of postpartum care in the U.S. contains additional fractures: no paid maternity leave, insurance coverage gaps including limits on postpartum coverage duration, Medicaid cuts and hospital closures expanding maternity care deserts, a severe shortage of perinatal mental health specialists, and racial bias that leaves Black and Brown mothers less likely to be heard, believed, or adequately treated.
A 2021 study of infanticide published in Psychiatry Online found that "myriad factors influence the outcome of these trials" and that "determining criminal responsibility and the guilt or innocence of a woman includes a wide range of inconsistencies." The study also found that women in the reproductive and postpartum period are in the most vulnerable stage of life for developing a mental illness and requiring psychiatric hospitalization.
What functional postpartum care requires
Real postpartum care, advocates argue, would include multiple postpartum check-ins, urgent access to perinatal mental healthcare, communication between every clinician prescribing medication, and one provider responsible for coordinating the whole picture. Other countries offer models: the Netherlands sends a trained maternity nurse to the home daily for the first 8 to 10 days, covered by standard insurance; the U.K. hands off care from midwife to health visitor to GP with checkpoints at 10 to 14 days and 6 to 8 weeks, plus a separate mental health check for the mother since 2020; and the WHO recommends four postnatal contacts in six weeks.
"If we continue treating cases like this as isolated, unimaginable tragedies—without confronting the fragmented, underfunded postpartum system surrounding them—we leave the same conditions in place for the next mother and the next family," concludes the analysis from Your Local Epidemiologist.
Clancy has pleaded not guilty. If the defense succeeds under Massachusetts' "lack of criminal responsibility" standard, she will be sent to a state psychiatric hospital. If it fails, she faces a life sentence.
