PSMA PET/CT-Guided Salvage Radiotherapy Shows Promising 5-Year Outcomes in Post-Prostatectomy Patients

A comprehensive retrospective analysis published in the Journal of the National Comprehensive Cancer Network demonstrates that prostate-specific membrane antigen (PSMA (search)) PET/CT-guided salvage radiotherapy produces favorable long-term outcomes for patients experiencing biochemical recurrence (search) following radical prostatectomy.
The study, which included 113 patients treated between January 2016 and May 2021, showed a median progression-free survival (PFS) of 49.2 months after a median follow-up of 59.4 months. The 2- and 5-year PFS rates were 72.6% and 48.7%, respectively, with confidence intervals of 64.3%-80.8% and 38.9%-58.6%.
Clinical Outcomes and Survival Metrics
The median freedom from distant metastases reached 76.4 months, with 85.8% of patients remaining metastasis-free at 2 years and 72.4% at 5 years. Overall survival outcomes were particularly encouraging, with the median not reached and 5-year survival rates of 97.1%.
Among the 48 patients who experienced radiographic progression, 42 were identified through PSMA (search) PET/CT imaging. Notably, only 2 patients experienced in-field progression, while 46 developed new metastatic sites, demonstrating the precision of the imaging-guided approach.
Patient Stratification and Treatment Optimization
The study revealed significant differences in outcomes based on PSMA (search) PET/CT staging. Patients with T0N0M0 disease showed the most favorable results, with an adjusted hazard ratio of 0.25 compared to the M1b/M1c cohort. Those with N1/M1a disease and TrN0M0 disease also demonstrated significantly improved PFS outcomes with adjusted hazard ratios of 0.39 for both groups.
"PSMA (search)-PET/CT scans allow us to see exactly where cancer is and tailor treatment accordingly," explained senior author Jeremie Calais, MD, PhD, director of the clinical research program in the department of Nuclear Medicine and Theranostics at UCLA. "Patients can get the therapy they need while avoiding unnecessary adverse effects, and even those with no visible disease can do very well with standard radiation."
Treatment Intensification Strategies
The analysis included prespecified secondary analyses examining the impact of whole-pelvis radiotherapy (WPRT) and androgen deprivation therapy (search) (ADT). Among the 113 patients, 55.8% received WPRT and 67.3% received ADT.
WPRT showed differential benefits depending on disease staging. While it did not significantly improve PFS among patients with T0N0M0 disease, it demonstrated a significant improvement in the TrN0M0 cohort with an adjusted hazard ratio of 0.12.
ADT was significantly associated with PFS improvement among patients with nodal or distant metastasis, showing an adjusted hazard ratio of 0.37. However, it did not provide significant benefits for patients with T0N0M0 or TrN0M0 disease.
Patient Demographics and Baseline Characteristics
The study population had a median age of 67 years at the time of PSMA (search) PET/CT imaging. Most patients presented with pathologic ISUP grade 3 disease (35.4%) and pathologic stage pT2N0/Nx disease (44.2%). Notably, 40.7% of patients had no visible lesions on PSMA PET/CT, while 38.9% had a single visible lesion.
The median initial PSA level was 8.0 ng/mL, and at the time of PSMA (search) PET/CT, the median PSA level was 0.4 ng/mL. The median time from radical prostatectomy to imaging was 19.9 months.
Clinical Implications and Future Directions
"This research underscores the value of incorporating PSMA (search)-PET/CT findings into clinical guidelines when deciding whether to add whole-pelvis radiation or hormone therapy to salvage radiation after prostate surgery," noted lead author John Nikitas, MD, a resident in the department of radiation oncology at UCLA Health (search).
The findings suggest a fundamental shift in managing biochemically recurrent prostate cancer (search), moving from anatomically-based consensus guidelines to biologically-informed, personalized treatment strategies. As noted by Edward Christopher Dee, MD, from Memorial Sloan Kettering Cancer Center, "PSMA (search) PET/CT enables unprecedented risk stratification, informs decisions about treatment intensification with whole-pelvis radiotherapy or ADT, and identifies oligometastatic disease amenable to consolidative therapy."
The authors acknowledged several limitations, including the non-randomized, retrospective design and lack of standardized treatment protocols. Several ongoing prospective studies, including the phase 3 PSMA (search)-SRT trial, the phase 2 PEACE-V STORM trial, the ADOPT trial, and the observational PSICHE trial, are expected to provide further validation of these findings.
