Radiotherapeutics Poised for Major Market Expansion as Industry Addresses Infrastructure Challenges

Theranostic radiopharmaceuticals, which combine diagnostic imaging with targeted radiation therapy, are emerging as "the future" of the field by enabling precise patient selection and treatment monitoring.
Major pharmaceutical companies including Bristol Myers Squibb (search), Eli Lilly, and AstraZeneca have invested billions in radiopharmaceutical acquisitions, with venture financing reaching a record $408 million in 2023.
Radioligand therapies are approaching a critical inflection point, with industry leaders projecting massive market expansion despite ongoing infrastructure challenges that must be resolved to realize the technology's full potential.
At the recent Sachs Oncology Forum, Geoff Towle, who heads Novartis' radioligand therapy business, revealed ambitious growth projections for the sector. "I believe my CEO has been on the record to say that he expects us to be at least $10 billion business as a market for Novartis alone," Towle stated. "And that's certainly part of our ambition."
Ben Hickey, President of RayzeBio (search), which was recently acquired by Bristol Myers Squibb (search), echoed this optimism. "It may not be the size of the I/O market, but there's no reason to believe that this can't be in the tens of billions of dollars," he said, explaining that Bristol's acquisition was motivated by adding another therapeutic modality to their existing portfolio.
Theranostic Approach Drives Innovation
The convergence of diagnostic and therapeutic capabilities in radiopharmaceuticals is creating new opportunities for precision oncology. Theranostic radiopharmaceuticals, which combine radioligands with radioactive isotopes visible through imaging scans, are being positioned as "the future" of the field.
"With theranostics, a physician can send a patient for imaging and see the extent of their disease. They can then precisely expose tumours to radiation through the radioactive payload, which will go to the exact locations picked up on the scan," explained Volker Wagner, CMO of French radiopharmaceutical biotech Orano Med (search).
This imaging capability provides significant advantages in drug development. "You can see in advance what will happen," said Perspective Therapeutics CEO Thijs Spoor. "It's like seeing the question and the answer to the exam before you take it. For drug development, it's a massive de-risk."
Major Pharmaceutical Investment Wave
The sector has attracted substantial investment from pharmaceutical giants. Between 2023 and 2024, Eli Lilly acquired Point Biopharma (search), AstraZeneca purchased Fusion Pharmaceuticals, and Bristol Myers Squibb (search) bought RayzeBio (search) through multi-billion dollar deals. Venture financing for radiopharmaceuticals reached a record high of $408 million in 2023, with a 330% venture capital surge in Q1 2024 compared to the first quarter of 2023.
Novartis has already demonstrated commercial success with two approved radioligand therapies. GlobalData forecasts that Lutathera will generate $1.1 billion and Pluvicto will bring in $5.1 billion for the company by 2031.
Current Development Landscape
According to Jack Hoppin, CEO of Boston's Ratio Therapeutics (search), the current state represents "the isotope wars," with various companies experimenting to find the most effective and safe isotopes. Most companies remain in pre-clinical stages, but Hoppin predicts rapid expansion once isotope selection is finalized.
Radioligand therapies offer advantages over other targeted approaches like antibody drug conjugates. "There're some inherent benefits to the delivery of radiation therapy," said Lori Lyons-Williams, CEO of Abdera Therapeutics (search). "You don't need to worry about some of the toxicities you have to think about with some of the payloads on the ADC front."
Infrastructure Challenges Remain
Despite the promising outlook, significant logistical hurdles must be addressed before widespread commercialization. Supply chain constraints for radioactive materials pose a major challenge, particularly given the short half-life of certain isotopes like actinium.
"Ultimately, part of the challenge is also in getting these to customers. Actinium has a relatively short half-life. And so, part of the incremental challenge is the infrastructure and investments needed to ensure that we can actually reliably get these to customers in a way that patients are able to show up to the office and actually receive them," Towle explained.
Imaging capacity represents another bottleneck. "The ability to provide the imaging, the PET scan capacity, these are real things," said Hoppin. "Personnel. There were 3.2 million PET procedures in 2025 in the United States of America. That's going to go to 6 million in four or five years."
Regulatory and Manufacturing Considerations
The use of radioisotopes adds complexity to the regulatory process, requiring coordination between drug and nuclear regulators. Dosimetry emerges as a critical consideration, helping determine appropriate dosing and potential side effects in healthy organs.
Manufacturing challenges include raw material availability, shipping complexity, and shortages of professionals with specialized expertise. However, contract development and manufacturing organizations (CDMOs) are increasingly entering the radiopharmaceutical sector to address these needs.
"CDMOs will likely play an ever-larger role in the clinical development and commercialisation of radiopharmaceuticals, as they will facilitate the expansion of companies without traditional field capabilities," noted Aryeh Sand, healthcare partner and nuclear medicine specialist at investment firm Solomon Partners.
Market Outlook
Industry leaders remain bullish about the sector's prospects despite current challenges. "As we project forward the next five to 10 years, that's where you're really able to see a massive inflection point for RLT," said Lyons-Williams. "The big opportunity is getting into these larger markets, getting into other targets, getting into earlier lines of therapy. And that's what will really be the fuel that will just put RLT into a different level, a different stratosphere, for how we see it as a modality in cancer."
The combination of strong efficacy signals, reduced toxicity profiles, and the ability to leverage existing antibody drug conjugate targets positions radioligand therapies for significant growth as infrastructure challenges are systematically addressed.
