Rare POLE Gene Mutation Predicts Strong Immunotherapy Response in Colorectal Cancer

Distinguishing Effective POLE Mutations
The research team, led by John Paul Shen, M.D., assistant professor in Gastrointestinal Medical Oncology, and Giulia Maddalena, M.D., Ph.D., sought to understand why some colorectal cancer (search) patients respond exceptionally well to immunotherapy (search) while others do not. While POLE mutations have been proposed as biomarkers for immunotherapy responses, it remained unclear which specific mutations were associated with clinical benefit.
"Not all POLE mutations behave the same. By refining how we classify POLE subtypes, we can better select patients who will experience the most meaningful benefit from immune checkpoint therapy (search)," Shen explained.
The LOP mutations affect specific functions of the POLE protein (search), which plays a crucial role in DNA replication and repair. These mutations create a distinct molecular signature that appears to make tumors highly susceptible to immune system recognition and attack.
Clinical Evidence and Patient Outcomes
The study retrospectively analyzed 69,223 patients across tumor types using data from cBioPortal and included a clinical cohort of 11 patients treated at MD Anderson. The analysis revealed that while 2.8% of tumors across cancer types carried POLE mutations, only 0.1% harbored the highly responsive LOP variants.
In the clinical cohort, nine colorectal cancer (search) patients with LOP POLE mutations demonstrated remarkable responses to immunotherapy (search), achieving an 88.9% overall response rate and 100% disease control rate. These patients experienced significant tumor shrinkage and durable responses. In contrast, patients with non-LOP POLE mutations did not experience similar benefits from immunotherapy treatment.
