Replicate Bioscience (搜索) has published promising eight-month durability data for its self-replicating RNA (srRNA) rabies (搜索) vaccine candidate RBI-4000, demonstrating superior immune persistence compared to traditional vaccines in a Phase I clinical trial. The results, published in Communications Medicine, a Nature portfolio journal, represent the first demonstration that an RNA vaccination can elicit protective immunity with comparable or superior durability to approved traditional vaccine platforms.
Extended Immune Response Duration
The Phase I randomized, open-label clinical trial (NCT06048770) enrolled 89 healthy volunteers aged 18-45 years and evaluated RBI-4000 across three dose levels: 0.1, 1, and 10 micrograms in prime-boost regimens, plus a single 10-microgram dose cohort. Following two administrations of RBI-4000 at 1 or 10 microgram dose levels, 100% of participants retained detectable rabies virus neutralizing antibodies (搜索) (RVNA) at 6 months, with 61% and 88% respectively maintaining antibodies above the WHO-defined metric of seroprotection for rabies (搜索).
Post-hoc analysis using eight different statistical decay models demonstrated that RBI-4000 generated RVNA responses of equal or superior durability compared to RabAvert, an approved inactivated rabies (搜索) vaccine comparator. Critically, half-lives of RVNA responses in RBI-4000 treatment cohorts were extended by at least 6-fold compared to the commercial inactivated vaccine cohort.
Low-Dose Efficacy and Safety Profile
All dose levels of RBI-4000 tested, including doses as low as 0.1 micrograms, elicited protective immune responses through Day 85. The majority of participants across all dose cohorts achieved the WHO-established threshold of protective RVNA response when administered in prime-boost or single-dose schedules. Trial participants were followed through 8 months after initial vaccination.
RBI-4000 demonstrated good tolerability throughout the study period, with no dose-limiting adverse events reported and no maximum tolerated dose reached. There were no serious adverse events or treatment-related discontinuations among any participants through study termination.
Addressing Global Health Challenge
Rabies (搜索) remains endemic in over 150 countries and presents a significant global health burden, with a 99% mortality rate if left untreated and causing 59,000 deaths annually. Current rabies vaccines require 3 to 5 doses and face supply shortages due to complicated manufacturing processes, limiting accessibility in resource-limited settings where the disease burden is highest.
