The reimbursement agreements include the provinces of Ontario, Alberta, British Columbia, Saskatchewan, and Nova Scotia, marking a crucial step in improving patient access to this specialized treatment for a rare genetic condition.
Clinical Significance for Rare Disease Patients
"These listing agreements for IMCIVREE mark a significant milestone for individuals living with obesity (search) due to Bardet-Biedl syndrome (search)," said Prof. Andrea M. Bardet-Biedl syndrome (search) is a rare autosomal recessive ciliopathy that presents with multiple symptoms evolving over time, including hyperphagia, early-onset severe obesity (search), visual impairment, polydactyly, genital abnormalities, renal dysfunction, and cognitive impairment.
Regulatory Pathway and National Strategy Integration
IMCIVREE received approval from Health Canada in May 2023 following priority review and was recently added to the common list of new drugs for rare diseases as part of the Government of Canada's National Strategy for Drugs for Rare Diseases. The Common Drug List ensures that the National Strategy provides the greatest possible benefit to all patients with rare diseases.

Mechanism of Action and Therapeutic Profile
IMCIVREE (setmelanotide) is an MC4R (search) agonist that targets the melanocortin-4 receptor pathway. In Canada, the drug is indicated for weight management in adult and pediatric patients 6 years of age and older with obesity (search) due to Bardet-Biedl syndrome (search) or genetically confirmed biallelic pro-opiomelanocortin (POMC (search)), proprotein convertase subtilisin/kexin type 1 (PCSK1 (search)), or leptin receptor (LEPR (search)) deficiency due to variants interpreted as pathogenic, likely pathogenic, or of uncertain significance.
Safety Profile and Monitoring Requirements
The most common adverse reactions with an incidence of 20% or greater include skin hyperpigmentation, injection site reactions, nausea, headache, diarrhea, abdominal pain, vomiting, depression, and spontaneous penile erection. Healthcare providers must monitor for several important safety considerations, including disturbances in sexual arousal, depression and suicidal ideation, hypersensitivity reactions, and skin hyperpigmentation with development of new melanocytic nevi.
The treatment requires full body skin examination prior to initiation and periodic monitoring during treatment to assess pre-existing and new pigmented lesions. Patients experiencing suicidal thoughts or behaviors, or clinically significant depression symptoms may require treatment discontinuation.
Global Regulatory Status
Beyond Canada, setmelanotide has received regulatory approval in multiple jurisdictions. The FDA has approved the drug for adult and pediatric patients aged 2 years and older with syndromic or monogenic obesity (search) due to BBS or genetically confirmed POMC (search), PCSK1 (search), or LEPR (search) deficiency. Both the European Commission and the UK's Medicines & Healthcare Products Regulatory Agency have authorized setmelanotide for similar indications in adults and children 2 years of age and above.
The drug is specifically not indicated for obesity (search) due to suspected POMC (search), PCSK1 (search), or LEPR (search) deficiency with variants classified as benign or likely benign, or for other types of obesity not related to BBS or the specified genetic deficiencies, including general polygenic obesity.
