A new retrospective study has found that the Risk Analysis Index (search) (RAI) provides significantly more accurate predictions of 30-day mortality and failure to rescue in patients undergoing surgical fixation for distal femur fractures (search) compared to traditional metrics such as age and the American Society of Anesthesiologists (ASA) classification.
Kush Modi and colleagues from the Georgetown University School of Medicine (search) analyzed 2,638 patients with isolated distal femur fractures (search) drawn from the ACS-NSQIP database. The study, published in the Indian Journal of Orthopaedics, compared the predictive performance of the RAI against the 5-Item Modified Frailty Index (mFI-5), age, and ASA scores using multivariate regression and AUROC analyses.
Superior Predictive Accuracy
The RAI demonstrated a C-statistic of 0.80 for 30-day mortality, outperforming both age (0.68) and the ASA score (0.73). For failure to rescue — defined as death within 30 days of a major complication — the RAI achieved a C-statistic of 0.75, compared to just 0.61 for the mFI-5.
The overall cohort experienced a 3.4% mortality rate and a 1.9% failure to rescue rate. Within this population, patients classified as very frail by the RAI faced more than six times the odds of 30-day mortality (OR 6.36) compared to their non-frail counterparts. Very frail individuals also had more than double the odds of experiencing failure to rescue (OR 2.57) and major complications (OR 2.40) following surgery.
Readmission Risk and Frailty Burden
Increasing frailty scores were significantly associated with higher odds of 30-day readmission. Very frail patients were nearly three times more likely to return to the hospital within 30 days (OR 2.87), underscoring the broader postoperative burden faced by this population.
A More Comprehensive Frailty Assessment
The RAI's superior performance is attributed to its 11-variable weighted assessment of physiologic reserve, which incorporates domains such as cognitive function and nutritional status — factors absent from the simpler mFI-5. This broader scope enables a more holistic evaluation of patient vulnerability beyond chronological age or anesthesia risk classification alone.
The study authors suggest that clinicians consider adopting the RAI as a standard bedside screening tool to identify high-risk patients early, enabling targeted multidisciplinary optimization strategies to improve surgical outcomes.
Limitations and Future Directions
The researchers acknowledge the study's limitations, including its retrospective database design and focus on 30-day outcomes. They highlight the need for future research examining whether preoperative prehabilitation and specialized geriatric care interventions can mitigate mortality risks in frail orthopedic populations.
