The approval comes at a critical time as 2024 recorded the highest number of dengue cases ever documented in a 12-month period, with over 14.6 million cases and more than 12,000 dengue-related deaths globally.
Clinical Performance and Technical Specifications
The Elecsys Dengue Ag (search) test is an in-vitro diagnostic immunoassay that qualitatively detects the NS1 antigen of dengue virus in human serum and plasma, serving as a key marker of acute infection during the first days of illness. Clinical studies demonstrated the test's robust performance with 94.90% sensitivity (CI: 91.44–97.26%) in PCR-confirmed positive samples and 99.96% relative specificity (CI: 99.79–100%) in a large cohort of healthy blood donors.
The test reliably detects NS1 antigen across all four dengue virus serotypes—DENV-1, DENV-2 (search), DENV-3 (search), and DENV-4 (search). It operates on Roche's fully automated cobas e immunoassay analyzers, including cobas e 801 systems and cobas e 402 platforms, which have an extensive installed base in laboratories worldwide.
Operational Advantages and Workflow Integration
The automated test delivers results within 18 minutes and offers throughput capacity ranging from 120 to 300 tests per hour, depending on the platform used. This high-throughput capability is designed to support effective patient management during outbreaks and enhance surveillance efforts in both endemic and non-endemic settings.

Dengue fever represents the most common mosquito-borne viral illness worldwide, affecting over 100 countries across all continents. The disease's symptoms are not unique, making it difficult for physicians to distinguish dengue from other fever-causing illnesses such as chikungunya or yellow fever. This diagnostic challenge underscores the importance of early and accurate testing to enable timely patient management and prevent severe complications.
Over the past 50 years, dengue spread has accelerated significantly, with half of the world's population now living in areas at risk of infection. The Elecsys Dengue Ag (search) test represents the first component of Roche's planned comprehensive Elecsys Dengue Panel. The complete panel will include the current NS1 antigen test alongside forthcoming Elecsys Dengue IgM and IgG tests. This integrated approach is designed to address all key serological biomarkers for dengue diagnosis, covering NS1 for acute infection detection, IgM for recent exposure identification, and IgG for long-term immunity assessment.
The World Health Organization has recognized the diagnostic value by including dengue virus NS1 antigen, along with other biomarkers, in its Essential Diagnostics List—a compilation of in-vitro diagnostic solutions that should be accessible to all countries to increase timely and life-saving diagnoses.
Disease Burden and Clinical Implications
Dengue is transmitted to humans through bites from infected female mosquitoes, primarily Aedes aegypti. While most patients experience no symptoms or only mild, self-limiting manifestations including fever, body aches, and headache, a small percentage can progress to severe dengue. This severe form is life-threatening and involves complications such as bleeding and organ impairment.
Children, elderly individuals, those with weakened immune systems, and patients with underlying conditions face higher risks of developing severe dengue. Close monitoring and fluid management remain crucial interventions to prevent severe complications in affected patients.
