Roche's Tecentriq Combination Shows Promise in Advanced Bladder Cancer Phase III Trial

Roche announced positive results from its Phase III IMvigor130 study, marking the first successful trial of a cancer immunotherapy combination in previously untreated advanced bladder cancer. The study showed that Tecentriq (atezolizumab) plus platinum-based chemotherapy significantly improved progression-free survival compared to chemotherapy alone in patients with locally advanced or metastatic urothelial carcinoma.
Study Results and Clinical Impact
The IMvigor130 trial demonstrated a statistically significant improvement in progression-free survival for the Tecentriq combination, with a median PFS of 8.2 months compared to 6.3 months for chemotherapy alone (hazard ratio=0.82, 95% CI: 0.70-0.96; p=0.007). This represents a meaningful clinical advancement for a patient population with limited treatment options.

While overall survival results showed promise, with a median OS of 16.0 months for the combination versus 13.4 months for chemotherapy alone, these findings did not achieve statistical significance at the interim analysis (HR=0.83, 95% CI: 0.69-1.00).
Safety Profile and Tolerability
The safety analysis revealed no new safety signals with the Tecentriq-chemotherapy combination. Grade 3-4 adverse events occurred in 85% of patients receiving the combination compared to 86% receiving chemotherapy alone. Treatment-related Grade 3-4 adverse events were reported in 83% versus 81% respectively, indicating no worsening of tolerability with the addition of Tecentriq.
Treatment discontinuation due to adverse events occurred in 11% of combination patients compared to 7% in the chemotherapy-alone arm, suggesting manageable tolerability profiles.
Study Design and Patient Population
IMvigor130 enrolled 1,213 patients with metastatic urothelial carcinoma who had not received prior systemic therapy for metastatic disease. The multicentre, partially blinded, randomised Phase III study included three arms: Tecentriq plus platinum-based chemotherapy, Tecentriq monotherapy, and platinum-based chemotherapy plus placebo as the control.
The study evaluated patients eligible and ineligible for cisplatin chemotherapy, using gemcitabine combined with either cisplatin or carboplatin as the chemotherapy backbone.
Market Context and Competitive Landscape
Tecentriq was the first cancer immunotherapy approved in advanced bladder cancer, and bladder cancer indications currently represent about 20% of the drug's sales. The positive results come amid increasing competition from Merck (search)'s Keytruda (pembrolizumab), which also holds approval for previously untreated patients and generates comparable absolute sales figures despite representing only 5% of Keytruda's total U.S. sales.
Both drugs face FDA restrictions limiting first-line use to patients whose tumors express PD-L1 (search), a biomarker correlated with improved immunotherapy responses. This restriction followed earlier data showing PD-L1-low patients receiving immunotherapy monotherapy experienced shorter survival than those given chemotherapy.
Unmet Medical Need
In 2018, over half a million new bladder cancer cases were diagnosed globally, with approximately 200,000 deaths from the disease. Urothelial carcinoma accounts for about 90% of all bladder cancer cases, with 30% considered advanced based on muscle-invasive or metastatic disease.
Despite improvements in tolerability, there have been no efficacy improvements in chemotherapy as standard of care for more than 30 years, leaving patients with poor outcomes and highlighting the significant unmet medical need in this population.
Next Steps
Roche plans to present complete study results at an upcoming medical meeting and share data with health authorities including the FDA. The company continues four ongoing Phase III studies evaluating Tecentriq alone and in combination with other medicines in early and advanced bladder cancer as part of its extensive development program across multiple cancer types.
