Roswell Park Researchers Develop First Genomic Tool to Guide Kidney Cancer Immunotherapy Decisions

The breakthrough study, published in Cancer Cell, emerged from investigating why a rare but aggressive kidney cancer (search) subtype shows unexpected susceptibility to immunotherapy.
Unlocking the Mystery of Sarcomatoid Kidney Cancer
The research focused on sarcomatoid renal cell carcinoma (search) (sRCC), an aggressive subtype that comprises 5% of all kidney cancer (search) cases. While typically diagnosed at late stages and resistant to most anticancer therapies, sRCC has shown disproportionate benefit from immune checkpoint blockade (search) (ICB) compared to clear cell renal cell carcinoma (search) (ccRCC), the most common kidney cancer type.
"We focused on the unexpected responsiveness of sarcomatoid renal cell carcinomas to checkpoint inhibition to better understand what can make kidney tumors more susceptible to immunotherapy (search) in general," said Nicholas Salgia, the study's first author and MD/PhD candidate at the Jacobs School of Medicine and Biomedical Sciences at the University at Buffalo.
Advanced Genomic Analysis Reveals Immune Advantages
Using state-of-the-art single-cell RNA sequencing technology, the research team analyzed tumor data from more than 3,000 kidney cancer (search) patients. Their analysis revealed that sRCC tumors possess a more robust immune system architecture compared to ccRCC tumors.
The key findings showed that sarcomatoid tumors contain a higher volume of plasma cells (search), which play a crucial role in producing antibodies that can tag cancer cells for destruction. Additionally, these aggressive tumors harbor more "immune hubs" called tertiary lymphoid structures (search), specialized areas where immune cells communicate with each other to coordinate anti-tumor responses.
Creating a Clinical Decision Tool
The research team, led by senior authors Jason Muhitch, PhD, Associate Professor and Co-Chair of the Genitourinary Translational Research Group, and Eric Kauffman, MD, Associate Professor of Oncology, developed a genomic dedifferentiation signature (GDS) to address a critical clinical gap. Until now, no reliable tool existed to determine which advanced kidney cancer (search) patients would benefit most from immunotherapy (search) versus targeted therapy.
"This tool holds promise as a biomarker for guiding decisions in advanced kidney cancer (search)," said Dr. Muhitch. "We identified a set of genes that were increased in these aggressive tumors, and from this set of genes, we created a novel gene signature that can identify patients with aggressive disease who are also primed to respond to immune-based cancer therapy."
Clinical Implications and Future Applications
Dr. Kauffman, who specializes in kidney cancer (search) care at Roswell Park, emphasized the potential clinical impact: "This signature may expose an Achilles' heel of sarcomatoid kidney cancers that makes them more vulnerable to immunotherapy (search) treatment. Our study lays the groundwork for developing future tests to help us better manage this disease, and its implications may also be applicable to other types of kidney cancer."
Next Steps in Clinical Validation
To translate these retrospective genomic findings into clinical practice, the Roswell Park team plans to initiate a prospective study within the next year. This study will assess the impact of the gene signature for predicting immunotherapy (search) response in kidney cancer (search) patients who have undergone nephrectomy.
The collaborative research involved scientists from multiple institutions, including the Translational Genomics Research Institute in Arizona, City of Hope Comprehensive Cancer Center in California, and Yale University School of Medicine in Connecticut.
