Ruxolitinib Combination Therapy Shows Promise in Checkpoint Inhibitor-Resistant Hodgkin Lymphoma

Scripps Research (搜索) scientists have identified a promising strategy to overcome checkpoint inhibitor resistance in cancer (搜索) patients by combining the approved immunosuppressive drug ruxolitinib with existing checkpoint therapies. The findings, published in Science on June 21, demonstrate that this unexpected combination can supercharge T-cell responses and significantly improve treatment outcomes for patients with refractory Hodgkin lymphoma (搜索).
Clinical Trial Results Show Dramatic Improvement
A phase I clinical trial conducted at the University of Minnesota evaluated 19 patients with Hodgkin lymphoma (搜索) who had either failed to respond to checkpoint inhibitors (搜索) or relapsed following initial treatment. The combination of ruxolitinib with nivolumab, a standard checkpoint inhibitor, produced remarkable results: 87% of patients remained alive two years after starting treatment, and 46% showed no signs of cancer (搜索) progression.
"Among patients with all types of cancer (搜索), fewer than 20% respond to checkpoint inhibitors (搜索). Even in cancer types that typically respond well such as Hodgkin lymphoma (搜索), about 10% to 20% of patients don't respond to checkpoint inhibitors," explains first author Jaroslav Zak, a postdoctoral fellow at Scripps Research (搜索). "It's very difficult to treat these patients."
The clinical outcomes represent a significant advance over conventional salvage chemotherapy options for this patient population. Unlike chemotherapy, which typically only slows disease progression, the combination therapy actually reversed cancer (搜索) in some patients, with at least one patient maintaining a strong response beyond the two-year trial period.
Mechanism of Action Through Myeloid Cell Modulation
The therapeutic success stems from ruxolitinib's ability to modulate myeloid cells (搜索), which cancer (搜索) often hijacks to promote tumor growth and suppress immune responses. The JAK (搜索) inhibitor works by targeting the JAK/STAT (搜索) pathway, a critical signaling cascade for immune cell development that becomes dysregulated in both inflammation and cancer.
"We're now enlisting myeloid cells (搜索) as helpers for immunotherapy, as it seems that in order for T cells (搜索) to increase in number and functionality, ruxolitinib needs to modulate the myeloid cells," explains co-senior author John Teijaro, PhD, professor in the Department of Immunology and Microbiology at Scripps Research (搜索).
The combination therapy resulted in reduced levels of myeloid suppressor cells and decreased neutrophil-to-lymphocyte ratios—both indicators associated with poor cancer (搜索) prognosis. Simultaneously, the treatment increased populations of functional T cells (搜索) and natural killer cells, key immune effectors in cancer surveillance and elimination.
Unexpected Immunoenhancing Properties
The findings were particularly surprising given ruxolitinib's established role as an immunosuppressive agent approved for treating chronic graft-versus-host disease. The drug's ability to enhance rather than suppress immune responses when combined with checkpoint inhibitors (搜索) challenges conventional understanding of JAK (搜索) inhibitor mechanisms.
"Ruxolitinib is actually an immunosuppressive drug that's clinically approved for chronic graft-versus-host disease, so the fact that we saw immune-enhancing effects in patients treated with this drug using combination therapy was definitely surprising," notes Zak. "This suggests that some drugs can actually have immune-enhancing effects, even if their primary indication is to relieve inflammatory disease pathology."
The research team identified ruxolitinib as a candidate through ReFRAME, a drug repurposing library developed by Calibr-Skaggs (搜索) that allows researchers to screen thousands of FDA-approved drugs for new therapeutic applications. Preclinical studies in mouse models with various cancers and viral infections confirmed that the ruxolitinib-checkpoint inhibitor combination increased both T cell and natural killer cell numbers compared to checkpoint therapy alone.
Future Clinical Development
Building on these promising results, researchers are planning expanded clinical trials to test the ruxolitinib combination in other cancer (搜索) types, including solid tumors. The team is also investigating whether other JAK (搜索) inhibitors might prove even more effective than ruxolitinib for cancer treatment.
"It's very rare to have supporting evidence with preclinical data and a clinical trial in one paper," says Teijaro. "Our results are particularly exciting because we are already seeing patients benefit from the combination and we believe this could be applied to several immunotherapy resistant cancers."
The research represents a significant step forward in addressing the substantial unmet need for effective treatments in checkpoint inhibitor-resistant cancers, potentially offering new hope for patients who have exhausted standard immunotherapy options.
