Semaglutide Shows Unprecedented Weight Loss Results in Major International Trial

A groundbreaking international trial has demonstrated that semaglutide, a drug currently used to treat type 2 diabetes (search), can produce unprecedented weight loss results in people with obesity (search). The study, published in the New England Journal of Medicine and involving nearly 2,000 participants across 16 countries, found that the drug helped patients achieve weight loss levels previously only attainable through surgical intervention.
Trial Results Show Dramatic Weight Loss
The 68-week trial revealed remarkable outcomes for participants receiving semaglutide at a 2.4mg weekly dose. On average, participants lost 15.3kg, with 75% of those taking the drug losing more than 10% of their body weight. Most significantly, 35% of participants lost more than 20% of their total body weight, compared to just 2.4% weight loss in the placebo group.
"The findings of this study represent a major breakthrough for improving the health of people with obesity (search)," said principal author Rachel Batterham, Professor of Obesity, Diabetes and Endocrinology who leads the Centre for Obesity Research at UCL and the UCLH Centre for Weight Management. "No other drug has come close to producing this level of weight loss - this really is a game changer."
Mechanism of Action and Clinical Significance
Semaglutide works by hijacking the body's own appetite regulating system (search) in the brain, leading to reduced hunger and calorie intake. The drug helps insulin get released from the pancreas (search) in patients with type 2 diabetes (search) and is currently sold under the brand name Ozempic at lower doses of around 1mg for diabetes treatment.
Dr. Robert F. Kushner, study author and obesity (search) researcher at Northwestern University Feinberg School of Medicine, described the drug as a "game-changer" and stated that "this is the start of a new era of effective treatments for obesity."
Additional Health Benefits Observed
Beyond weight loss, the trial demonstrated significant improvements in cardiovascular and metabolic health markers. Participants experienced reductions in risk factors for heart disease (search) and diabetes, including decreased waist circumference, blood fats, blood sugar, and blood pressure. Participants also reported improvements in their overall quality of life.
Professor Batterham emphasized the broader health implications: "The impact of obesity (search) on health has been brought into sharp focus by COVID-19 (search) where obesity markedly increases the risk of dying from the virus, as well as increasing the risk of many life-limiting serious diseases including heart disease (search), type 2 diabetes (search), liver disease (search) and certain types of cancers."
Study Design and Participant Demographics
The international trial was conducted across 129 sites in 16 countries, with participants receiving either 2.4mg of semaglutide or placebo weekly. Both groups followed a 500-calorie deficit diet and were encouraged to exercise for 150 minutes per week. The study authors noted that white women were overrepresented in the study, raising questions about the drug's effectiveness across different populations.
Safety Profile and Limitations
While the results were promising, the trial revealed some safety concerns. Side effects included nausea and diarrhea, which occurred more frequently in the semaglutide group. Additionally, 23 people taking the drug developed gallstones, compared to four in the placebo group.
The 68-week trial duration also raised questions about long-term effectiveness. Trial participant Qiana Mosely, who lost 40 pounds during the study, told the New York Times she regained weight after the trial ended, stating "I was so sad."
Regulatory Pathway and Future Implications
Professor John Wilding from the University of Liverpool, the trial's UK Chief Investigator, noted that semaglutide is already approved and used clinically at lower doses for diabetes treatment, making doctors familiar with its use. "So it is good to see this translated into an effective treatment for people with obesity (search)," he said.
With evidence from the international trial, semaglutide has been submitted for regulatory approval as an obesity (search) treatment to the UK's National Institute of Clinical Excellence (NICE), the European Medicines Agency (EMA), and the US Food and Drug Administration (FDA).
Dr. Eduardo Grunvald, medical director of the weight management program at UC San Diego Health, who was not involved in the study, called the results "very exciting" and noted that "it's a major advancement in the field because we have never had a medication that has demonstrated this degree of weight loss."
The international trial was funded by pharmaceutical company Novo Nordisk, and if approved for obesity (search) treatment, insurance coverage may vary depending on whether insurers view obesity as a lifestyle choice or a medical disease.
