A new study analyzing Medicare claims data suggests that certain diabetes medications may offer unexpected benefits for older adults beyond blood sugar control, potentially helping to preserve strength and mobility as patients age.
The research, published in Diabetes Care, examined older adults with type 2 diabetes (search) who were starting new diabetes medications and tracked their frailty (search) progression over one year. The findings revealed that patients beginning treatment with SGLT-2 (search) inhibitors such as empagliflozin (Jardiance) and dapagliflozin (Farxiga), or GLP-1 receptor (search) agonists such as semaglutide (Ozempic) and liraglutide (Victoza), showed slower frailty progression compared to those starting other diabetes medications.
Study Design and Methodology
Researchers analyzed a national 7% sample of U.S. Medicare claims, focusing on older adults who were just starting a new diabetes medicine. They compared people who began GLP-1 receptor (search) agonists, SGLT-2 (search) inhibitors, DPP-4 (search) inhibitors, or sulfonylureas, using a validated claims-based frailty (search) index (CFI) that ranges from 0 to 1, with higher scores indicating greater frailty.
The team tracked one-year changes in this frailty (search) index to assess how different medications affected the progression of weakness, slowed movement, and fatigue in older diabetic patients.
Key Findings
Compared with new users of DPP-4 (search) inhibitors, those starting GLP-1 receptor (search) agonists had a mean change in frailty (search) index of about -0.007 (95% CI: -0.011 to -0.004), while those starting SGLT-2 (search) inhibitors showed a mean change of about -0.005 (95% CI: -0.008 to -0.002). In contrast, users of sulfonylureas showed no significant difference from the DPP-4 inhibitor group.
"While frailty (search) still tended to worsen with age, it progressed more slowly in those groups," the researchers noted. Importantly, when they accounted for cardiovascular events and other safety issues, these factors explained only a small part of the benefit.
Clinical Implications
The study's findings are particularly significant given that roughly 10-15% of adults over age 65 experience frailty (search), with higher rates among those with type 2 diabetes (search). People with diabetes face increased frailty risk due to chronic inflammation, muscle loss, cardiovascular disease, and the cumulative burden of managing a complex condition.

Direct Effects Beyond Cardiovascular Benefits
The fact that cardiovascular and safety events accounted for only a small portion of the observed benefit suggests that GLP-1 and SGLT-2 (search) medicines may have direct effects on pathways linked to frailty (search), beyond their known heart and kidney benefits. This finding could represent a paradigm shift in how clinicians approach medication selection for older diabetic patients.
Study Limitations and Future Directions
The researchers acknowledge that this observational study based on claims data cannot prove that the medicines themselves slow frailty (search), and the size of the effect is modest at the individual level. The study relied on a claims-based frailty index rather than clinical examination, which represents a limitation in the assessment methodology.
Despite these limitations, the findings add to the growing understanding that when choosing diabetes medicines for older adults, clinicians should consider factors beyond blood glucose control alone. For older persons with type 2 diabetes (search), especially those already at risk of frailty (search), a GLP-1 or SGLT-2 (search) medicine, where suitable and affordable, may help preserve strength, mobility, and independence as part of a broader care plan that includes activity, nutrition, and social support.
