A new study published in Diabetes Care reveals that sodium-glucose cotransporter 2 (search) inhibitors (SGLT2i) may significantly reduce the medication burden for patients managing both gout (search) and type 2 diabetes (search). The research, led by Dr. Natalie McCormick from Mass General Brigham (search)'s Rheumatology and Allergy Clinical Epidemiology Research Center, found that SGLT2i use was associated with a 38% lower probability of starting allopurinol compared to dipeptidyl peptidase 4 (search) inhibitors (DPP-4i).
Study Design and Patient Population
The researchers conducted a population-based target trial emulation study analyzing over 18,000 patients diagnosed with both gout (search) and type 2 diabetes (search). All participants filled new prescriptions for glucose-lowering treatments—either SGLT2i, DPP-4i, or glucagon-like peptide 1 receptor (search) agonists (GLP1-RA (search))—and were not currently using urate-lowering therapies (ULTs).
The study team used statistical techniques to balance baseline characteristics between treatment groups, including demographics and number of recent gout (search) flares, effectively mimicking randomization. Patients were followed over time to compare the probability of starting ULTs, rates of gout flares, and prescriptions for gout flare treatments and diuretics.
Clinical Significance and Unmet Need
Gout (search) affects 5.1% of U.S. adults, representing over 12 million people, with prevalence reaching 10% among those aged 65 or older. The condition frequently co-occurs with cardiovascular-kidney-metabolic conditions, with one in four U.S. adults with gout also having type 2 diabetes (search).
"Conventional urate-lowering therapies are recommended for flare prevention, but they don't help with the CKM comorbidities, making it necessary for patients with co-occurring conditions to take medications in different classes to treat both," McCormick explained. "This makes treatment decisions more complex, while increasing pill burden for patients and the potential for drug interactions."
Key Findings
Beyond the 38% reduction in allopurinol initiation probability, patients using SGLT2i demonstrated several additional benefits:
- Fewer prescriptions for NSAIDs, colchicine, and high-dose glucocorticoids
- Reduced frequency of gout flares
- Fewer prescriptions for diuretic medications, particularly loop diuretics
These findings remained consistent across different sensitivity analyses, including comparisons between SGLT2i and GLP1-RA (search), and were validated in an external dataset.
Mechanism and Clinical Implications
SGLT2 inhibitors (search), already approved for treating type 2 diabetes (search), heart failure (search), and chronic kidney disease (search), have been shown to decrease serum urate levels and reduce the need for diuretic medications—a major risk factor for gout (search) flares. The reduction in gout-related medication use is particularly significant given that patients with gout face elevated risks for cardiovascular, gastrointestinal, kidney, and metabolic adverse effects from flare medications.
"Our findings hold promise for those living with gout (search) and type 2 diabetes (search)," McCormick noted. "SGLT2i use could reduce their exposure to NSAIDs (search) and glucocorticoids (search), which often have adverse effects, by lowering their use of gout-related medications."
Future Research Directions
The study opens new avenues for investigating polypharmacy in gout (search) patients. McCormick highlighted that polypharmacy, typically defined as regularly taking at least five medications, "hasn't been well studied in gout, so that's an avenue I'd like to pursue further in the future."
The research suggests that SGLT2 inhibitors (search) may offer a dual therapeutic benefit, addressing both glucose control and gout (search) management while potentially streamlining care and reducing overall medication burden for this complex patient population.
