Shionogi Secures $482 Million BARDA Contract to Strengthen U.S. Biodefense Against Drug-Resistant Bacterial Threats

Shionogi (search) Inc., the New Jersey-based subsidiary of Japanese pharmaceutical company Shionogi & Co., Ltd., has been awarded a significant contract through the Biomedical Advanced Research and Development Authority's (BARDA (search)) Project BioShield program. The contract, initially funded at $119 million with multiyear options totaling up to $482 million, focuses on Fetroja (cefiderocol) as a critical countermeasure against difficult-to-treat Gram-negative bacterial infections and high-priority biothreat pathogens.
Strategic National Security Initiative
The contract represents a substantial investment in strengthening U.S. domestic manufacturing capabilities and supply readiness for antimicrobial countermeasures. BARDA (search)'s Project BioShield accelerates the research, development, procurement and availability of effective medical countermeasures against chemical, biological, radiological and nuclear (CBRN) agents.
The funding will enable Shionogi (search) Inc. to establish a U.S. drug product manufacturing site for Fetroja, support procurement activities, and advance the antibiotic for treating infections caused by high priority biothreat pathogens, including Burkholderia pseudomallei (search) (melioidosis (search)) and Yersinia pestis (search) (plague (search)). Additionally, the contract will support expanding Fetroja's utility for hospital-acquired bacterial pneumonia (search) and ventilator-associated bacterial pneumonia (search) (HABP/VABP) in pediatric patients through a U.S. Food and Drug Administration (FDA) Supplemental New Drug Application (sNDA).
Novel Antibiotic Mechanism
Fetroja represents the world's first siderophore cephalosporin antibiotic, marking a significant innovation in antimicrobial therapy. In the United States, the FDA has approved Fetroja for treating hospital-acquired bacterial pneumonia (search), ventilator-associated bacterial pneumonia (search), and complicated urinary tract infections (search) caused by certain susceptible Gram-negative microorganisms in patients 18 years of age or older.
The drug is indicated for complicated urinary tract infections (search), including pyelonephritis, caused by Escherichia coli, Klebsiella pneumoniae, Proteus mirabilis, Pseudomonas aeruginosa, and Enterobacter cloacae complex. For HABP/VABP, Fetroja is approved for infections caused by Acinetobacter baumannii (search) complex, Escherichia coli, Enterobacter cloacae complex, Klebsiella pneumoniae, Pseudomonas aeruginosa, and Serratia marcescens.
Clinical Safety Profile and Monitoring
Clinical studies have revealed important safety considerations for Fetroja. An increase in 28-day all-cause mortality was observed in Fetroja-treated nosocomial pneumonia, bloodstream infections, or sepsis patients compared to those treated with best available therapy (BAT) in clinical study NCT02714595. Most BAT regimens contained colistin, and all-cause mortality remained higher in patients treated with Fetroja than in patients treated with BAT through Day 49.
Deaths generally occurred in patients with infections caused by Gram-negative organisms, including non-fermenters such as Acinetobacter baumannii (search) complex, Stenotrophomonas maltophilia, and Pseudomonas aeruginosa. The deaths resulted from worsening or complications of infection, or underlying comorbidities, though the cause of the increase in mortality has not been established.
The most common adverse reactions in complicated urinary tract infection trials included diarrhea (4%), infusion site reactions (4%), constipation (3%), rash (3%), candidiasis (2%), cough (2%), elevations in liver tests (2%), headache (2%), hypokalemia (2%), nausea (2%), and vomiting (2%). In HABP/VABP trials, the most frequent adverse reactions were elevations in liver tests (16%), hypokalemia (11%), diarrhea (9%), hypomagnesemia (5%), and atrial fibrillation (5%).
