Southampton Researchers Launch Clinical Trial Using Diagnostic Tampons for Early Ovarian Cancer Detection

The VIOLET study, led by Dr. Jemma Longley, a consultant medical oncologist at University Hospital Southampton (搜索), represents a novel approach to cancer detection that could enable women to screen for ovarian cancer (搜索) from their own homes. The trial is funded by The Eve Appeal (搜索) and conducted in collaboration with the Southampton Clinical Trials Unit (搜索) and the Centre for Cancer Immunology (搜索) at the University of Southampton.
Addressing Critical Screening Gap
Currently, no screening program exists for ovarian cancer (搜索), leaving healthcare providers to rely on patients presenting with symptoms. According to Cancer Research UK statistics, only around 27% of ovarian cancer cases are diagnosed in early stages, with an average of 21 women diagnosed daily with the disease.
"There is currently no screening programme available for ovarian cancer (搜索)," Dr. Longley explained. "Women with recognised genetic mutations, such as BRCA (搜索), may opt to have surgery to remove their ovaries and fallopian tubes to reduce their risk of developing the disease. However, this can have significant long-term health effects by putting younger women into a surgical menopause."
The challenge is compounded by the vague nature of ovarian cancer (搜索) symptoms, which often overlap with other conditions. As Dr. Victoria Goss, Associate Professor of Early Diagnosis at the Southampton Clinical Trials Unit (搜索), noted: "Symptoms of ovarian cancer can often crossover with other conditions. If we have these biomarkers of the early stages of cancer, that means that when patients are presenting potentially with vague symptoms like a bit of abdominal pain, some weight loss, then these biomarkers could be used to figure out whether or not that patient actually does have cancer."
Innovative Diagnostic Approach
The VIOLET study involves collecting vaginal fluid samples from women undergoing surgery for suspected or confirmed ovarian cancer (搜索) and comparing them with samples from healthy volunteers. The research team will analyze these samples for changes in microRNAs - tiny biological signals present in both ovarian cancer cells and vaginal fluid.
Patients participating in the trial receive a diagnostic tampon pack containing two tampons. They are instructed to insert one tampon between 20 minutes and eight hours within the 24 hours prior to their surgery. On the day of surgery, researchers collect the tampon along with additional samples including vaginal swabs, blood tests, urine tests, and any tissue remaining after surgery.
Dr. Longley emphasized the accessibility advantages of this approach: "At the moment, the focus of early detection is on blood based markers. And that puts the onus on the participant or the patient to go and find a health professional to do that blood test. You have to be registered with the GP surgery. You have to make time for that. There's a lot of women that are very busy these days with jobs and children and things. I wanted to figure out how we could make a screening test that could be done in your own home that was community based and was accessible to all."
Patient Perspective and Clinical Impact
The research has garnered support from patients who understand the urgent need for improved early detection. Alison, a 65-year-old retired psycho-oncology nurse from Southampton who was diagnosed with ovarian cancer (搜索) at age 40, expressed enthusiasm for the study.
"I was very lucky enough to see a great team at the Princess Anne Hospital and luckily, my ovarian cancer (搜索) was caught early, which is quite unusual," Alison said. "There haven't been that many advances in ovarian cancer detection or treatment over the years. And, you know, sadly, a significant number of women are still dying despite some new drugs. So I think anything like this, which is a reasonably easy study to do and take part in, is really exciting."
Alison emphasized the critical nature of early detection: "Far too many women are still diagnosed too late. I've lost friends, sadly, who were diagnosed at stage three and four. So, something like this new study with the tampon is very exciting."
Future Applications and Expansion
The potential implications of this research extend beyond ovarian cancer (搜索) detection. Dr. Longley outlined the broader vision for the diagnostic tampon technology: "This could be a massive game changer for women diagnosed with ovarian cancer and also other cancers of the gynaecological tract. We could open this up to different cancers - so cervical cancers and endometrial cancers and potentially create a multi-cancer early detection test with a diagnostic tampon."
The five-year study is currently recruiting patients at University Hospital Southampton (搜索), with plans to expand recruitment to additional locations. The research is supported by the Southampton Experimental Cancer Medicine Centre and biotechnology company Daye (搜索), alongside funding from The Eve Appeal (搜索).
