A new study presented at the American College of Gastroenterology (search) (ACG) 2024 Scientific Sessions reveals that specialized staffing strategies and care management approaches can significantly expedite access to advanced therapies for inflammatory bowel disease (search) (IBD (search)) patients.
The research, conducted at Capital Digestive Care (search)'s 28-provider community-based practice, analyzed 233 patients who started new biologic treatments in 2023. The study found that practices with dedicated prior authorization coordinators achieved remarkably efficient processing times, with a mean time to insurance approval of just 4.03 days and an average of 14.17 days to treatment initiation.
Impact of Insurance Appeals and Care Management Models
The study highlighted significant disparities in approval times based on insurance requirements. Patients who faced insurance appeals experienced substantial delays, with approval times extending to 19.31 days compared to just 3.09 days for those without appeals (P <.001). This six-fold increase in waiting time underscores the substantial impact of administrative hurdles on treatment initiation.
A particularly noteworthy finding emerged regarding chronic care management homes. Patients treated through these specialized units experienced dramatically shorter drug start times - 8.6 days compared to 14.85 days in standard care settings. This nearly 50% reduction in time-to-treatment demonstrates the potential benefits of dedicated care management approaches.
Patient Demographics and Treatment Pathways
The study population was predominantly White (86%) and female (63.4%). Researchers found no significant correlations between approval times and various factors including disease diagnosis, therapy line, age, race, sex, or insurance type (commercial vs federal).
Treatment delivery methods showed varying efficiency levels. Patients receiving infusion therapy through buy & bill arrangements experienced shorter waiting periods (11.65 days to first infusion) compared to those using specialty pharmacy channels (17.56 days).
Administrative Burden and Practice Implications
"Our overall approval times and drug start times are relatively fast, suggesting that having dedicated and trained prior authorization coordinators plays a role in getting patients on advanced therapy sooner, though increases overhead for our practice," noted the research team led by Amy Stewart, NP.
The findings emphasize the critical role of specialized administrative support in expediting IBD (search) care, despite the additional operational costs. However, the researchers also highlighted ongoing challenges, stating that "Prior authorization requirements continue to place significant administrative and financial burden on practices, lack transparency and ultimately delay patient care."
