Surgical Resection Combined with EGFR TKI Shows Promising Results in Metastatic NSCLC Trial

A novel combination approach of surgical resection following EGFR (search) tyrosine kinase inhibitor therapy has demonstrated promising progression-free survival benefits in patients with EGFR-mutant metastatic non-small cell lung cancer (NSCLC), according to results from a randomized phase II trial presented at the International Association for the Study of Lung Cancer 2025 World Conference on Lung Cancer.
The study, led by Dr. Pei-Hsing Chen of National Taiwan University Hospital, represents the first trial to evaluate primary tumor resection as local consolidation therapy with EGFR (search) tyrosine kinase inhibition in both oligometastatic and polymetastatic NSCLC patients.
Novel Treatment Strategy Shows Significant Benefit
The trial enrolled 91 patients with oligometastatic and polymetastatic EGFR (search)-mutant advanced NSCLC who received 12 weeks of afatinib treatment before randomization. Patients were then assigned in a 1:1 ratio to either continue afatinib therapy or undergo primary tumor resection with or without radiotherapy for distant metastases.
The surgical intervention achieved a hazard ratio for progression of 0.48 (95% confidence interval: 0.25-0.93; P = 0.031), though the researchers noted that the data remain immature and continued follow-up is necessary.
"Our study explores whether targeting residual disease through surgery can extend the benefits of EGFR (search) tyrosine kinase inhibition beyond standard monotherapy," Dr. Chen explained. "The purpose of surgery is not curative, but rather to serve as part of a combination therapy to prolong the interval before drug resistance develops."
Pathologic Response Rates and Molecular Insights
The surgical approach yielded notable pathologic responses, with major pathologic responses observed in 29.4% of patients (10/34) and pathologic complete responses in 5.9% (2/34). However, major pathologic response has not yet correlated with improved survival outcomes.
Subgroup analysis revealed interesting patterns based on EGFR (search) mutation types. Patients with EGFR exon 19 deletions in the surgical arm demonstrated higher major pathologic response rates compared with patients harboring EGFR L858R mutations. Conversely, a lower hazard ratio for progression-free survival was observed in patients with L858R mutations compared to those with exon 19 deletions.
Molecular Profiling Reveals Additional Complexity
Next-generation sequencing analysis of postoperative tissue samples from 30 patients provided valuable molecular insights. The analysis revealed TP53 (search) mutations in 36.6% of patients and co-mutations in 50% of cases. While the hazard ratios for TP53 mutations and co-mutations were 1.4 and 1.7 respectively, these results did not reach statistical significance.
The surgical approach offers a unique advantage by providing access to postoperative pathologic and molecular data, which could inform future patient selection strategies and treatment optimization.
Clinical Implications and Future Directions
This groundbreaking study challenges the traditional treatment paradigm for metastatic NSCLC by demonstrating that surgical intervention can serve as an effective component of combination therapy rather than solely as a curative measure. The approach aims to target resistant subclones and reduce overall tumor burden to extend the duration of disease control beyond what EGFR (search) tyrosine kinase inhibition alone can achieve.
