Targeting Inflammation in Depression
The four-week proof-of-concept trial enrolled 30 participants with moderate-to-severe depression (search) who had not responded well to standard antidepressants and showed signs of low-grade inflammation in two separate blood tests taken two weeks apart. Participants were randomly assigned to receive either tocilizumab (14 people) or a placebo (16 people).

However, approximately one in three people with depression do not improve with these conventional treatments. The research revealed that baseline high-sensitivity C-reactive protein (hs-CRP) levels, rather than IL-6 levels, better tracked depression (search) improvement, suggesting hs-CRP may serve as a more reliable predictor of immunotherapy response in depression than drug-specific biomarkers.
Safety and Tolerability
Tocilizumab was well tolerated throughout the trial, with no serious adverse events or patient withdrawals reported. This safety profile aligns with the drug's established use in treating autoimmune conditions.
The next phase will involve conducting a large-scale phase III randomized controlled trial to provide definitive evidence that could enable doctors to prescribe immunotherapy for depression (search).
