TransMedics Group (search) announced that the U.S. Food and Drug Administration has granted full approval of its Investigational Device Exemption for the Next-Generation OCS ENHANCE Heart trial, marking a significant milestone in organ transplant technology development. The approval follows the company's January 2026 FDA approval for its DENOVO Lung IDE trial and builds upon conditional IDE approval for the ENHANCE Heart trial received in August 2025.
Groundbreaking Trial Design
The ENHANCE trial represents an ambitious two-part clinical investigation that could reshape heart transplantation practices. Part A focuses on supporting prolonged heart perfusion using the OCS Heart System (search), while Part B is designed to demonstrate the superiority of OCS Heart perfusion in donation after brain death cases compared to static cold storage methods currently used in transplant centers.
The trial's scope is unprecedented in the field of heart preservation. With a total sample size expected to exceed 650 patients across both parts, TransMedics believes this will constitute the largest heart preservation for transplant trial ever conducted worldwide. Part B specifically targets the potential expansion of OCS Heart clinical indications to include standard criteria DBD hearts that are currently transplanted within 4 hours of preservation.
