The executive order, signed on August 13, instructs the Office of the Assistant Secretary for Preparedness and Response (search) (ASPR) within the Department of Health and Human Services (search) to compile a list of approximately 26 critical drugs and stockpile a six-month supply of their active pharmaceutical ingredients (search) (APIs). The ASPR has been given 30 days to prepare this list.
Addressing Critical Supply Chain Vulnerabilities
The initiative comes as the US faces significant pharmaceutical supply chain challenges, with 60% of finished medicines and 90% of APIs currently produced overseas. Of the manufacturers that produce APIs used in FDA-approved products, only 11% are based in the United States, while around 43% of branded pharmaceutical APIs come from the European Union.
"Restoring capacity for domestic production of essential pharmaceutical products is essential to safeguarding national health and security against global supply chain disruptions," a White House spokesperson stated.
The executive order emphasizes that "stockpiling APIs is advantageous as APIs are generally lower-cost and have longer shelf lives than the finished drug products they make." It also claims that creating the stockpile will "insulate the US from the concentration of foreign, sometimes adversary, nations" involved in supplying the starting materials used to make APIs.
Building on COVID-19 Era Infrastructure
The SAPIR was originally established in 2020 during Trump's first term in response to the COVID-19 (search) pandemic. The reserve was designed to maintain nearly two years' worth of medicine supply for the most important FDA-approved treatments. However, the White House claims the reserve has remained near-empty for the past few years due to a lack of investment, blaming the Biden administration for failing to advance domestic production or fill the SAPIR "despite spending billions on supply chain initiatives."
The current executive order also instructs the ASPR to update a 2022 list of 86 essential medicines and develop a plan for acquiring and stockpiling a six-month supply of their APIs. The existing SAPIR repository is expected to be ready to begin receiving and storing API supplies within 120 days, with a second repository planned for establishment a year later.
Industry Response and Manufacturing Commitments
The pharmaceutical industry has already begun responding to Trump's broader domestic manufacturing strategy. Major pharmaceutical companies including AstraZeneca, AbbVie (search), Roche, Novartis, Eli Lilly, and Johnson & Johnson have committed to US manufacturing investments, encouraged by the threat of pharma sector-specific tariffs and efforts to reduce regulatory barriers.
AbbVie (search) recently announced that its $10 billion pledge to boost US domestic manufacturing includes a $195 million investment in a new API facility at its North Chicago campus.
Regulatory Support for Domestic Manufacturing
The executive order aligns with broader regulatory efforts to support domestic pharmaceutical production. The FDA recently introduced the PreCheck programme on August 7, which comprises a two-phase approach designed to streamline the introduction of new pharmaceutical manufacturing facilities in the United States.
The executive order specifies that APIs sourced and stored in the program should come from domestic manufacturers "where possible," with the HHS budget covering the project's costs. However, the administration acknowledges that reconfiguring pharma's complex supply chains will be challenging within a single presidential term.
