Norm Krimbill was diagnosed with stage IV esophageal cancer (search) in April 2021 after experiencing difficulty swallowing. An endoscopy revealed a tumor at the junction of the esophagus and stomach, while a PET scan showed the cancer had metastasized to lymph nodes, spine, and ribs. With surgery and radiation ruled out due to extensive disease spread, Krimbill enrolled in a clinical trial led by Dr. Sunnie Kim examining the addition of oral tucatinib to standard treatment.
Novel Combination Approach
The trial protocol combines tucatinib with chemotherapy (search) plus Herceptin (trastuzumab), a targeted therapy that blocks HER2 (search) protein activity. "Herceptin binds to the receptor on the outside of the cell, and tucatinib works on the receptor on the inside of the cell, so they impact the receptor in different ways to create a more complete response," Leal explained.
Exceptional Treatment Response
After just a few treatments, Krimbill's metastatic disease disappeared, leaving only the primary esophageal tumor. While the cancer will never be fully cured, it has remained stable without growth since treatment initiation. This outcome represents a dramatic improvement over historical expectations.
"The average lifespan of someone with metastatic disease with standard care, if you look at the data, was around a year-and-a-half at that time," Leal noted. "He started with us in April of 2021, and here we are in 2026, and he still is on the first-line treatment."
Quality of Life Maintenance
Krimbill has received 115 infusions over four years while maintaining excellent quality of life. He continues biweekly treatments with minimal impact on daily activities, recently completing a three-week vacation to Hawaii. The treatment regimen has been well-tolerated, with the patient reporting that treatment breaks during vacations help clear chemotherapy (search) from his system, allowing him to feel "really good."
"He tolerates the treatment incredibly well, and he hasn't had any detriment to his quality of life," Leal confirmed.
