Unapproved Retatrutide Vials Found With Double the Labeled Dose, Sparking Overdose Warnings

University of Queensland (search) researchers and harm reduction groups are conducting further testing to assess the risks of black-market peptides, which remain unregulated and easily accessible online.
A laboratory analysis of an unapproved weight loss peptide sold online has found that a vial labeled as containing 10 mg of retatrutide actually held 19 mg — nearly double the stated concentration — prompting urgent warnings from physicians about the "enormous" risk of overdose.
The testing, conducted by a Melbourne laboratory accredited by the National Association of Testing Authorities, was arranged and paid for by researchers at the University of Queensland (search)'s School of Public Health and harm reduction organization PED Test Australia as part of an ABC investigation. The "research use only" vial was acquired from an unregulated internet site.
"The potential for overdose is enormous," said Melbourne-based gastroenterologist Darcy Holt. He described treating a patient who took a high dose of retatrutide and presented with "very serious" side effects, including a critical case of secretory diarrhoea that could be "life-threatening."
"He could have become so dehydrated with such severe electrolyte abnormalities that he could have died," Dr. Holt said.
The testing showed the vial had 16.6% purity, with the laboratory advising the remainder was likely "fill material," including substances such as mannitol, a sugar that stabilizes and protects active ingredients. Results for any contaminants in the product are still pending.
A Growing Black Market for Unapproved Peptides
Retatrutide is an investigational molecule owned by US-based pharmaceutical company Eli Lilly and is currently undergoing clinical trials for type 2 diabetes (search), obesity (search), and related complications. It has not been approved by any worldwide drug regulator, including the US Food and Drug Administration or Australia's Therapeutic Goods Administration (TGA).
"Retatrutide is an investigational molecule that has not been approved by any regulator anywhere in the world," an Eli Lilly spokesperson told the ABC. "No-one can legally sell it for human use."
Despite this, the drug is easily available via the online black market, driven in part by social media influencers promoting its supposed benefits. The attraction of "reta," as it is commonly called on social media, is that it may be faster-acting than existing prescription weight-loss drugs.
Associate research fellow Benjamin Bonenti, part of the University of Queensland (search) team researching the harms of certain peptides, said the testing revealed a "big problem."
"Double the dose of these types of drugs is a massive difference … it shows the market cannot be taken at face value," Mr. Bonenti said. "We can say that it is exposing [people who use peptides] to some potentially serious health risks."
He noted that retatrutide belongs to the same broad class as Ozempic-style weight loss drugs and acts on multiple hormone pathways involved in appetite, metabolism, and blood sugar. "To double the dose of a drug acting on these pathways, we know for certain with similar drugs like steroids, that it can have drastic effects on the body."
Clinical Consequences of Overdose
Dr. Holt, a gastroenterologist focusing on the digestive system, detailed the case of a young man who escalated his retatrutide dose and experienced severe secretory diarrhoea requiring hospitalization and extensive intravenous fluid rehydration.
"He was having loose, watery, large-volume bowel actions every one to two hours, including overnight, and losing what we estimated to be eight litres of fluid a day," Dr. Holt said. "He was really unwell, and I think if he hadn't been in hospital, then he would be in a very bad way."
These findings follow a separate ABC report in which senior doctors from a hospital in Melbourne's north warned of multiple cases of liver damage and other serious illness linked to substances labeled as retatrutide. Victoria's chief health officer issued a health alert stating the department was aware of six cases of acute liver toxicity associated with the unapproved peptide product.
Liver transplant specialist Marie Sinclair, from the Victorian Liver Transplant Unit based at the Austin Hospital, described cases including a woman in her 20s who presented with acute liver failure (search) after purchasing vials labeled as retatrutide online on a recommendation from her gym.
"We don't know what these things are, it's quite frankly terrifying," Dr. Sinclair said.
The Patient Perspective
The vial tested by the Melbourne laboratory was purchased by a woman who asked to remain anonymous. She told the ABC that a friend suggested she buy retatrutide from an online supplier because her GLP-1 prescription had become "cost-prohibitive." She said retatrutide use was "normalised" in her social circles.
"I guess it's just quite enticing, because you're talking to people who are using it who are like, 'Oh my God, it's like transformative, and my skin's amazing, and I've lost all this weight,'" she said.
When the vial arrived, however, she "chickened out" of using it. "My self-preservation kicked in … I respect the medical profession, and I can certainly see the risks."
Mr. Bonenti said the University of Queensland (search) researchers hoped to improve public education through their research and testing of unapproved peptides. "[The data] can influence policy and do things like ensure there is increased public awareness, increased testing … it forms the basis of the science that is known about these risks," he said. "It's a critical piece of harm reduction in this area."
Australia's Chief Medical Officer and the TGA have warned Australians not to take unapproved peptides, saying they are often poorly labeled and present a health risk. In March, the TGA issued a safety warning about counterfeit weight loss drugs, advising people to stick to approved, prescription weight loss medicines.
