Principal investigator Heather Armstrong, associate professor of gastroenterology at the University of Alberta, explains the tool's fundamental approach: "The goal is to understand, based on the microbes that live in your gut, are there specific types of carbohydrates that are best for your health? Are there certain types of carbohydrates that your microbes can't really do anything with, so you're either not going to receive a benefit or you may actually experience gut symptoms like bloating, gas, pain or discomfort?"
The system focuses on nondigestible carbohydrates—fibers and starches that aren't normally absorbed directly into the digestive tract but are instead broken down by microbes through fermentation. For IBD (search) patients, these carbohydrates often trigger symptoms, making personalized dietary guidance crucial for disease management.
Clinical Trial Design and AI Integration
Trial participants submit stool samples, which undergo analysis by culturing gut microbes with different types of carbohydrates. An AI algorithm then processes this data to create individualized nutrient profiles with specific recommendations on which foods to eliminate and which to incorporate. The testing protocol includes reassessment every three to six months to evaluate microbiome changes and adjust dietary recommendations accordingly.
Armstrong reports significant clinical improvements: "It's quite incredible from an IBD (search) point of view. We've actually seen increased rates of remission just by introducing these subtle changes in diet. We're finding it only takes a couple of weeks or a month to really rewire the microbiome."
Dietary Recommendations and Mediterranean Diet Principles
The precision diet tool provides specific guidance on carbohydrate selection based on individual microbiome capabilities. For patients whose gut microbiota cannot process beta fructans—found in chicory root, onions, garlic, and bananas—the system recommends reducing intake of these foods while suggesting alternatives such as citrus fruits high in pectin fibers, or potatoes, oats, beans, and lentils.
Armstrong emphasizes the importance of consuming whole citrus fruits, including peels and pith, which contain pectin loaded with phenolics—chemicals with anti-inflammatory properties. This approach aligns with Mediterranean diet principles often overlooked in North America. "You'll see whole citrus eaten in Albania and Greece and other Mediterranean nations. They'll take a whole lemon, cut the ends off, then cut up the whole thing and put it into a salad or mixed veggies or a stir fry," notes Armstrong, who has ulcerative colitis (search) herself.
Expanding Applications Beyond IBD
The precision diet tool is being tested in additional patient populations, including individuals with non-alcoholic liver disease (search), multiple sclerosis (search), and a control group of firefighters with normal digestion. Results from healthy participants have revealed unexpected benefits beyond digestive health.
"Some of our healthy controls who don't have IBD (search) are coming back to us saying, 'I'm no longer taking my statins, my blood pressure is reduced, I'm sleeping better at night or I've never felt better in my life,'" Armstrong reports.
Research Funding and Timeline
The clinical trials are supported by a $10-million Microbiome Big Bet grant from the Weston Family Foundation (search). Armstrong's research program operates with 17 active grants from organizations including the Canadian Institutes of Health Research, the Natural Sciences and Engineering Research Council of Canada, Canada Research Chair, Crohn's and Colitis Canada (search), and the Weston Family Foundation.
Armstrong anticipates clinical availability of the precision diet tool within three years, pending successful completion of the ongoing validation trials. The approach emphasizes whole food consumption over supplements, based on the principle that "the human gut is designed to receive certain nutrients at a certain point along the GI tract and at the same time as certain other nutrients that come with eating a whole food."
