Breakthrough Results in Implanted VNS Trial
The study, conducted by scientists from UT Dallas (搜索)' Texas Biomedical Device Center (搜索) in collaboration with the Baylor Scott & White Research Institute, paired traditional prolonged exposure therapy with concurrent delivery of short bursts of vagus nerve stimulation (搜索) via a small implanted device in participants' necks. Dr. Michael Kilgard, the Margaret Fonde Jonsson Professor of neuroscience, emphasized the unprecedented nature of these results.
"In a trial like this, some subjects usually do get better, but rarely do they lose their PTSD (搜索) diagnosis. Typically, the majority will have this diagnosis for the rest of their lives," Kilgard said. "In this case, we had 100% loss of diagnosis. It's very promising."
The trial represents the largest clinical trial to date using an implanted device for PTSD (搜索) treatment. After a standard 12-session therapy course, assessments were performed four times during the six months following treatment conclusion, with benefits persisting throughout this period for all participants.
Technology Evolution and Safety Profile
The VNS (搜索) technology builds on 13 years of research that has demonstrated the ability to accelerate neuroplasticity through paired stimulation. This work previously resulted in FDA approval for treating impaired upper-limb movement in stroke patients. Dr. Robert Rennaker, professor of neuroscience and the Texas Instruments Distinguished Chair in Bioengineering, has developed increasingly sophisticated versions of the implantable device.
"The technology we have is above and beyond anything else that's out there. The device is about 50 times smaller than our version from just three years ago," Rennaker said. The most recent wireless version is approximately the size of a dime, and the 49 people in the Dallas area with implanted devices have accumulated 100 years of combined experience with no reported issues.
Parallel Development of Non-Invasive Approach
Simultaneously, electroCore's gammaCore (搜索) non-invasive vagus nerve stimulation (搜索) device has entered PTSD (搜索) clinical testing, with the first eight of a planned 40 adults enrolled in a 12-week study at Acacia Clinics (搜索). This pilot study, conducted in collaboration with the Vagus Nerve Society (搜索), aims to generate preliminary safety and effectiveness data for an adjunctive PTSD indication.
The trial's primary safety endpoint focuses on treatment-related serious adverse events, while the primary efficacy endpoint measures change from baseline in CAPS-5 total score at week 12. The non-invasive approach offers potential advantages in terms of accessibility and reduced surgical risk.
Addressing Unmet Medical Needs
Current PTSD (搜索) treatments face significant limitations. Licensed clinical psychologist Dr. Mark Powers, lead author of the UT Dallas (搜索) study and research center director of the Trauma Research Center at BSWRI, noted that existing gold-standard treatments achieve approximately 85% response rates, with only 40% of patients no longer meeting diagnostic criteria and 20% dropout rates.
"VNS (搜索) has changed my work dramatically," Powers said. "Soon we could have the option of VNS for people who don't get better with cognitive behavioral therapy alone."
The National Center for PTSD (搜索) estimates that women are twice as likely to develop PTSD at some point in their lives, and many patients fail to respond to therapy or pharmacological intervention, or experience intolerable side effects or relapse.
Clinical Trial Progression
A double-blind, placebo-controlled Phase 2 pilot study for the implanted VNS (搜索) approach is currently ongoing in Dallas and Austin. This represents the next step toward potential FDA approval for a treatment that would be invented, tested, and delivered by UT Dallas (搜索), following the successful model established for stroke rehabilitation.
Dr. Seth Hays, associate professor of bioengineering and director of preclinical research at the TxBDC, highlighted the collaborative nature of the research: "It's been an incredibly rewarding experience to see this technology evolve from early discovery experiments in the lab to clinical benefits in patients."
