Vanda Pharmaceuticals Inc. has submitted a Biologics License Application (BLA) to the U.S. Food and Drug Administration for imsidolimab, a novel IgG4 IL-36 receptor (搜索) antagonist designed to treat generalized pustular psoriasis (搜索) (GPP). The submission represents a significant milestone for addressing this rare, life-threatening autoinflammatory skin disorder that affects an estimated 2 to 124 cases per million people worldwide.
Clinical Trial Results Support Regulatory Filing
The BLA is supported by positive results from the global Phase 3 GEMINI-1 and GEMINI-2 studies. In the 45-patient GEMINI-1 trial, patients were randomized 1:1:1 to receive a single intravenous infusion of 750 mg imsidolimab, 300 mg imsidolimab, or placebo on Day 0.
At the primary endpoint of Week 4, 53% of patients receiving either dose of imsidolimab achieved a Generalized Pustular Psoriasis (搜索) Physician Global Assessment (GPPGA) score of 0/1, indicating clear or almost clear skin. This compared to only 13% of patients in the placebo group, with the 750 mg dose achieving statistical significance (p=0.0131).
Sustained Long-term Efficacy Demonstrated
Responders from GEMINI-1 were re-randomized in GEMINI-2 to monthly subcutaneous maintenance dosing with 200 mg imsidolimab or placebo, followed for up to 116 weeks. All patients receiving active maintenance therapy maintained clear or almost clear skin with no flares throughout the approximately 2-year study period. In contrast, only 25% of patients in the placebo maintenance group maintained their response, while 63% experienced disease flares.
Addressing Significant Unmet Medical Need
GPP is characterized by sudden flares of widespread pustules, erythema, and systemic symptoms such as fever and fatigue. The condition is driven primarily by loss-of-function mutations in IL36RN (搜索), which leads to deficiency in the endogenous IL-36RA regulator. Imsidolimab works by inhibiting IL-36 receptor (搜索) signaling, directly addressing this underlying pathophysiology.

Across both GEMINI studies, imsidolimab demonstrated a favorable safety profile with no clinically meaningful safety signals, no treatment-related serious adverse events, and no discontinuations due to adverse events.
Vanda has requested priority review for the BLA, citing GPP's status as a rare orphan disease with significant unmet medical need. If granted, priority review would establish a six-month review cycle, potentially enabling FDA approval of imsidolimab for GPP treatment as early as mid-2026.
Strategic Portfolio Expansion
The imsidolimab development builds on Vanda's growing expertise in rare orphan disorders and anti-inflammatory portfolio, which includes Ponvory®, approved for treating relapsing forms of multiple sclerosis (搜索) and currently in clinical development for psoriasis (搜索) and ulcerative colitis (搜索). Regulatory and patent exclusivity for imsidolimab is expected to extend into the late 2030s, with Vanda holding an exclusive global license from AnaptysBio (搜索) for the drug's development and commercialization.
