The absence of a standardized definition for long COVID (搜索) is creating significant obstacles in clinical practice and research, according to a comprehensive study published in JAMA Network Open that analyzed multiple definitions across major health organizations and research studies.
The study, led by Lauren Wisk, PhD, from the University of California, Los Angeles, compared long COVID (搜索) definitions from the Innovative Support for Patients With SARS-CoV-2 (搜索) Infections Registry (INSPIRE) and five comparator studies, revealing substantial variations that impact both patient care and scientific understanding of the condition.
Dramatic Variation in Prevalence Rates
When applying different long COVID (搜索) definitions to the same patient population, researchers found prevalence rates ranging from 0.84% to 42.01% among COVID-19 (搜索)-positive individuals and from 28.08% to 40.32% among COVID-19-negative participants. This 50-fold difference in prevalence estimates underscores the urgent need for diagnostic standardization.
The analysis included data from 4,575 participants in the INSPIRE cohort who completed three-month follow-up surveys, with 3,521 testing positive for COVID-19 (搜索). Among participants with available race/ethnicity data, 68.9% were non-Hispanic White, 13.8% were Asian, 8.2% were Black, and 9.1% were multiracial or of other races.
Five Core Symptoms Identified
Despite the definitional chaos, researchers identified five symptoms consistently present across all studies:
- More tired than usual
- Pain or tightness in chest
- Diarrhea (more than three loose stools in 24 hours)
- Decreased smell or change in smell
- Fatigue, tiredness, or exhaustion
However, the symptom lists used for defining long COVID (搜索) across the five studies accounted for only 24.8% to 32.9% of variation, indicating that symptom selection significantly influences diagnostic outcomes.
Clinical and Economic Impact
The definitional uncertainty has real-world consequences for patients and healthcare systems. A separate study using the same INSPIRE cohort data found that 27.1% of 4,119 participants reported current long COVID (搜索) symptoms. These individuals were significantly more likely to experience reduced work hours, periods of unemployment, and attribute work loss to their symptoms compared to those who never had long COVID (71.1%) or had resolved symptoms (1.8%).
"There is a need for more robust literature to better understand the influence of specific long COVID (搜索) phenotypes on work and financial challenges, as well as interventions to both prevent and treat long COVID," said Michael Gottlieb, MD, lead author of the work impairment study.
Healthcare utilization patterns also reflect the condition's burden. Research published in BMC Public Health found that 17% of long COVID (搜索) patients were hospitalized at least once before diagnosis, and 40% visited emergency departments. While acute care utilization decreased over time, outpatient services increased by an average of 4.5 percentage points, contributing to healthcare costs estimated at upwards of $3.7 trillion over five years.
Diagnostic Challenges and Treatment Gaps
The National Academies of Sciences, Engineering, and Medicine (搜索) (NASEM) definition notably includes individuals who test negative for COVID-19 (搜索) but report similar symptoms. While this approach aims to address healthcare access disparities, it creates potential for misdiagnosis.
"If we don't have a good way to actually tell someone has long COVID (搜索), it's really hard to know if the current treatment protocols we've developed are going to do well for them," Wisk explained. "And if we're giving them a treatment for long COVID, we might be missing an opportunity to treat what's actually causing their symptoms."
The study found that multiple long COVID (搜索) definitions showed lower sensitivity (identifying fewer true cases) but higher specificity (correctly classifying individuals without the condition). A key distinguishing feature was overall symptom count, with long COVID patients presenting more symptoms than those with other post-illness conditions.
Vulnerable Populations
The CDC (搜索) identifies several groups at higher risk for long COVID (搜索): women, Hispanic and Latino individuals, those with severe initial COVID-19 (搜索) illness, people with underlying health conditions, adults 65 years or older, and unvaccinated individuals. However, the lack of standardized definitions makes it difficult to accurately assess risk factors and prevalence in these populations.
Call for Standardization
Researchers emphasize that establishing a universally accepted definition is crucial for advancing both clinical care and research. "It is crucial to establish a universally accepted definition of long COVID (搜索) that is informed by comprehensive research and that is both sensitive and specific," the study authors noted.
The World Health Organization released an International Classification of Diseases (ICD-10) code for long COVID (搜索) in 2021, but research has shown inconsistencies in coding practices and adherence to the outlined definition.
"We're not necessarily trying to say that any one definition that we looked at should be used," Wisk said. "We were comparing across the ones that exist to sort of see how they stack up, and I think, really importantly, what the implications are for our ability to actually capture people who believe that they have long COVID (搜索)."
The researchers advocate for policies supporting long COVID (搜索) patients, including extended leave, workplace accommodations, work-from-home options, and reduced hours. The 2022 Biden administration (搜索) response to long COVID addressed surveillance, prevention, and public awareness but did not fully describe the role of public health organizations in addressing the condition.
As the medical community continues to grapple with long COVID (搜索)'s complexities, the need for diagnostic clarity becomes increasingly urgent for the millions of Americans potentially affected by this condition.
