Study Design and Methodology
The single-center, randomized, crossover study involved 24 participants with moderate opioid experience, defined as at least 10 uses within the previous year and at least 3 uses within the previous 12 weeks for nontherapeutic purposes. Researchers used an opioid-induced respiratory depression (搜索) (OIRD) model where participants received carefully controlled intravenous fentanyl infusions that reduced breathing rates by approximately 50 percent from baseline.
Under standardized conditions and careful monitoring, each participant received both ZURNAI (搜索) (1.5 mg nalmefene delivered intramuscularly) and intranasal naloxone in separate sessions. Researchers tracked minute volume (MV) - the volume of air in liters breathed per minute - along with oxygen levels and vital signs to evaluate the speed and extent of respiratory depression (搜索) reversal.
Superior Pharmacodynamic Performance
ZURNAI (搜索) demonstrated clear advantages over intranasal naloxone across multiple measures. The onset of respiratory depression (搜索) reversal occurred between 2.5 to 5 minutes following ZURNAI administration. At the critical 5-minute timepoint, participants showed a breathing increase of 4.59 liters per minute with ZURNAI, compared to only 1.99 liters per minute with standard intranasal naloxone.
The superior performance continued at subsequent timepoints, with ZURNAI (搜索) maintaining consistent effects at 10, 15, 20, and 30 minutes after administration. Pharmacokinetic analysis revealed that nalmefene achieved a median time to peak concentration of 12.5 minutes versus 40 minutes for intranasal naloxone.
Extended Duration of Action
The study revealed significant differences in drug persistence, with nalmefene showing a half-life of 7.98 hours compared to naloxone's 1.64-hour half-life. ZURNAI (搜索) demonstrated higher mean plasma concentrations and extended duration compared to intranasal naloxone, supporting its potential utility in addressing overdoses from potent, long-acting synthetic opioids.

Safety Profile and Considerations
Both treatments were well-tolerated with no serious treatment-emergent adverse events reported. The most commonly reported side effects included feeling hot, nausea, headache, taste distortion, and sensitivity to touch.
However, the study highlighted important safety considerations for ZURNAI (搜索) use. Given nalmefene's greater potency as an opioid receptor (搜索) antagonist, it may carry a risk of prolonged precipitated withdrawal in individuals who are frequent opioid users and physically dependent on or tolerant to their effects. Healthcare providers must also remain aware that recurrence of respiratory depression (搜索) remains possible.
Clinical Implications and Availability
ZURNAI (搜索) is indicated for emergency treatment of known or suspected opioid overdose (搜索) induced by natural or synthetic opioids in adults and pediatric patients aged 12 years and older. The auto-injector format is designed for use across various settings, from hospitals and clinics to schools, homes, and other public places.

The company plans to make ZURNAI (搜索) available in late 2025, distributing the product for no profit as part of its commitment to help abate the opioid crisis. This approach aligns with the National Institutes of Health's support for developing more potent and longer-acting opioid antagonists to address the ongoing crisis.
Study Limitations and Future Research
Researchers acknowledged several study limitations, including insufficient session duration to document the full time-course of reversal effects. The controlled experimental setting, where fentanyl was infused over extended periods and titrated to maintain consistent plasma concentrations, may not reflect real-world overdose scenarios.
The authors emphasized that further research is needed to expand understanding of ZURNAI (搜索)'s performance in diverse real-world community settings, where overdose presentations and circumstances vary significantly from controlled clinical conditions.
