ZYMFENTRA Shows Consistent Efficacy Across Crohn's Disease Locations in Post-Hoc Analysis of LIBERTY-CD Study

Celltrion has announced the publication of a post-hoc analysis from the LIBERTY-CD study demonstrating that ZYMFENTRA (search) (infliximab-dyyb), a subcutaneous formulation of infliximab, maintains consistent efficacy across different disease locations in patients with moderately-to-severely active Crohn's disease (search). The findings, published in Clinical Gastroenterology and Hepatology, address a significant clinical challenge in treating ileal Crohn's disease.
Study Design and Patient Population
The post-hoc analysis examined endoscopic data from the phase 3 LIBERTY-CD trial, evaluating clinical outcomes and predictive factors by disease location in 329 patients receiving subcutaneous infliximab maintenance treatment. Among the study population, 52.6% had colon-dominant Crohn's disease (search) (n=173) while 47.4% had ileum-dominant disease (n=105).
Clinical Efficacy Outcomes
At Week 54, patients receiving subcutaneous infliximab achieved significantly superior efficacy outcomes compared to placebo across all endpoints, regardless of disease location. Clinical remission rates demonstrated the treatment's effectiveness: 60.95% in the ileum-dominant group and 66.95% in the colon-dominant group, compared to 37.25% and 29.09% respectively in placebo groups.
Clinical response rates similarly favored subcutaneous infliximab over placebo in both location groups, with 62.86% versus 41.18% in ileum-dominant disease and 67.80% versus 38.18% in colon-dominant disease. Endoscopic response rates showed comparable benefits: 53.33% versus 19.61% in ileum-dominant and 52.54% versus 18.18% in colon-dominant groups.
Segment-Level Analysis
When assessed at the individual segment level, endoscopic responses at Week 54 were consistently greater with subcutaneous infliximab versus placebo across all ileocolonic segments, including the challenging terminal ileum. The analysis revealed consistent treatment effect sizes throughout each segment, supporting the drug's broad therapeutic utility.
Clinical Significance
"The consistency of treatment effect across disease locations is important, as ileal Crohn's disease (search) is highly correlated with negative long-term outcomes and the fact that the terminal ileum being the most challenging segment to treat," said Prof. Jean-Frédéric Colombel, MD, from the Janowitz Division of Gastroenterology at Icahn School of Medicine at Mount Sinai.
The findings carry particular clinical relevance given that 70-80% of patients with Crohn's disease (search) have ileal involvement at diagnosis. "Effective treatment of the ileum has the potential to benefit a large proportion of patients with Crohn's disease," noted Nam Lee, Vice President of Global Medical Affairs at Celltrion.

ZYMFENTRA (search) is indicated for maintenance treatment of moderately-to-severely active Crohn's disease (search) and ulcerative colitis (search) in adults following initial treatment with an intravenous infliximab product. The drug works by blocking tumor necrosis factor-alpha (search) (TNF-alpha (search)), a protein that can trigger immune system attacks on healthy tissue when overproduced.
The FDA approved ZYMFENTRA (search) through a stand-alone Biologics License Application under the 351(a) pathway, recognizing it as a new biologic with first-approved subcutaneous administration. The drug maintains patent protection for its dosage form until 2037 and for its route of administration until 2040.
