入选标准
- •Consenting adult inpatients age > 18 years on treatment for at least 6 months period with 2 or more select chronic diseases such as
- •a)Cardiovascular diseases (hypertension, coronary artery disease, peripheral artery disease, stroke, chronic kidney disease, heart failure)
- •b)Musculoskeletal disorders (rheumatoid arthritis, osteoarthritis)
- •c)Chronic respiratory disorders (COPD, asthma)
- •d)Diabetes mellitus
排除标准
- •a)Patients who have psychiatric disorders, cognitive impairment or visual disturbance
- •b)Patients who are unlikely to follow up at 6 months (Eg: living in another location)
- •c)Critically ill patients
- •d)Patients who have cancer, HIV or TB
- •e)Patients who donâ??t know languages such as English, Kannada, Tamil, Telugu, Hindi and Marathi
- •f)Patients or care givers without mobile phones or unable to read English messages
