Arthroscopic shoulder surgery commonly requires regional anesthesia for analgesia but interscalene block is associated with a high incidence of hemi diaphragmatic paresis and reduction in respiratory function. This randomized controlled trial will compare combined infraclavicular and suprascapular nerve block with interscalene block in adult patients undergoing elective arthroscopic shoulder surgery. After written informed consent, eligible ASA I and II patients will be randomly allocated to receive either ultrasound guided combined infraclavicular plus suprascapular block or ultrasound guided interscalene block before surgery. Diaphragmatic excursion by ultrasound, spirometry values, pain scores at rest and on movement, analgesic consumption and adverse events will be recorded at predefined time points up to twenty four hours postoperatively. The primary objective is to evaluate hemi diaphragmatic sparing, and the secondary objectives are to compare analgesic efficacy, respiratory function and incidence of complications between the two techniques

- •Age 18 to 60 years ASA grade I and II Patient undergoing elective arthroscopic surgery.
排除标准
- •patients with pre-existing diaphragmatic paralysis, severe pulmonary disease, coagulopathies, allergy to local anaesthetics, local site infection and Patient refusal / not giving consent for undergoing procedure.
