入选标准
- •Thai patient is 30 to 70 years old
- •Previously diagnosed as Diabetes by a doctor and are currently taking diabetes medication
- •estimated glomerular filtration rate greater than or equal to 60 ml / min / 1.73 m2
- •must be treated prior at the Sub-district health office
排除标准
- •Known case of Autosomal Dominant Polycystic Kidney Disease, lupus nephritis, obstructive uropathy, kidney stone, nephrotic syndrome or primary glomerular disease
- •Metastatic cancer (estimated life expectancy of less than 2 years)
- •Communication problems such as dementia, deafness
