- •To capture a variety of patients/carers journeys we aim to have representation from a number of patient/carer groups including: Those referred directly from an ED; Those referred after a long period of time as an inpatient >10 days from our major referring specialties being general medical, orthopaedic rehabilitation, stroke/neurology and stroke rehabilitation; Those referred after a short period of time as an inpatient <10 days from our major referring specialties being general medical, orthopaedic rehabilitation, stroke/neurology and stroke rehabilitation.
排除标准
- •Patient’s/carers will be excluded based on the patient’s ability to participate such as: Moderate to severe dysphasia; Moderate to severe dementia; Inability to communicate in more than one domain (ie. written and spoken); Significant mental health presentation; Unsafe home environment for research staff.; Elective surgery
