- •First Ever stroke
- •Ischemic or Intracerebral Heamorrhagic stroke
- •Presence of uncontrolled Hypertension BP more than 140/90
排除标准
- •Life expectancy less than six months.
- •Refuse to consent
- •Patient with no stable home.
- •Patient unable to take medications by self-due to severe neurological deficits and/or no family member/caregiver is available to maintain continuous and timely delivery of medication
