•Aged 60 years or older.
•Diagnosed with first-ever ischaemic stroke confirmed by a physician.
•Medically stable and preparing for hospital discharge.
•Able to communicate in Thai.
•Six Item Cognitive Impairment Test (6CIT) score under
•National Institutes of Health Stroke Scale (NIHSS) score 0 to
•Able to use the LINE application independently or with assistance from a caregiver.
•Willing to provide written informed consent.

•History of recurrent stroke.
•Severe cognitive impairment or inability to provide informed consent.
•Severe aphasia or communication problems preventing participation.
•Severe psychiatric illness.
•Unstable medical condition requiring intensive treatment.
•Participation in another intervention study that may influence the study outcomes.
