•PHNs are eligible to participate if they are located in the state of Victoria, Australia, the Chief Executive Officer (CEO) of relevant PHNs or their delegate agrees to study participation, and 50% or more of general practices within their catchment area contribute de-identified patient data to the POpulation Level Analysis and Reporting (POLAR) database.
•Practices and GPs will be included if they work within the catchment area of participating PHNs and have agreed to the use of their de-identified POLAR data for research purposes as part of their agreement with Outcome Health for POLAR usage.
•De-identified POLAR data will be the source of the trial outcomes. Secure use of these data is covered by existing data governance agreements for PHN-approved research and quality improvement evaluation purposes.
•Patients are not being recruited.

•GPs who do not work in one of the participating PHNs.
•GPs who do not work in a general practice that contributes de-identified patient and other clinical data to the POLAR database.
•GPs or the general practice in which they work, have opted out of sharing de-identified POLAR data for research purposes.
•Patients are not being recruited.
•There are no other specific exclusions.
