•Adult (aged > 18-70 years old) patients with
•Lp(a) more than 200 nmol/L or
•Lp(a) more than 125 nmol/L (estimated around 60 mg/dL) and having ASCVD or having high risk for atherosclerosis cardiovascular disease (defined as Thai CV risk more than or equals to 20%)

•Having contraindication of heparin use such as hypersensitivity to heparin, history of heparin induced thrombocytopenia, active or uncontrolled bleeding, platelet count < 100,000/mm3 , coagulopathy
•Recent ASCVD or major surgery within 3 months
•Receiving PCSK9 inhibitor
•Having terminal illness
•Cannot provide consent by themselves
•History of allergy or reactions to the dialyzers
•Hypocalcemia (serum total calcium, corrected with albumin < 8.5 mg/dL) on the morning of scheduled LA
•Hypokalemia (serum potassium < 3.0 mmol/L) on the morning of scheduled LA
•Hypomagnesemia (serum magnesium < 1.6 mg/dL) on the morning of scheduled LA
•Severe anemia (Hct <25%)
