- •Malnutrition assess by Nutrition Alert Form (NAF) class B or C

- •Terminally ill patients with an end-of-life care plan
- •Patients who are already consulted with the clinical nutrition team.
- •Unable to obtain bodyweight or height
- •Body mass index >= 30 kg/m2
- •Patients who are admitted for planned procedure
