- •- Living (Semi) independently (in a sheltered home, completely independently)
- •- Presented at SEH with a fall-related complaint
- •- (in the VUmc) to be linked to a trained professional (i.e. living in the vicinity of a trained primary care provider, insured for the required care or willing to pay for this yourself)
排除标准
- •- Admission to a hospital or other healthcare institution
- •- Restrictive injury
- •- Unable to give informed consent
- •- Insufficient command of the Dutch or English language
- •- Patients who already participate in a fall prevention project Admitted to a care institution (i.e. rehabilitation clinic, care home)
- •- Patients living outside the Amsterdam region
