简要总结

This is a randomized controlled trial with a mixed method design to determine the impact of coaching on self-perceived medical errors, burnout, and resilience. The study team developed a novel coaching curriculum based in principles of positive psychology and self-reflection with the hypothesis that the coaching intervention will lead to decreased medical errors, decreased burnout, and increased resilience in trainee and faculty participants. Resident and fellow trainees as well as faculty members were recruited across departments and randomized to coaching or control. Faculty in the coaching arm were trained in coaching techniques and paired with a trainee coachee. Survey results as well as focus groups will be used to analyze the impact of the coaching program as compared to standard mentorship (control).

研究组 & 干预措施

Trainees - treatment

Experimental

Residents and fellows paired with a faculty coach from the "faculty - treatment" arm to participate in up to 4 coaching meetings

干预措施: Coaching (Behavioral)

Trainees - control

No Intervention

Residents and fellows randomized to the control arm. They are not paired with a faculty coach and instead continue to receive standard mentorship as part of their training program.

Faculty - treatment

Experimental

Faculty members randomized to receive coaching training and are paired with a resident/fellow from the "trainees - treatment" arm to conduct up to 4 coaching sessions over the course of the academic year.

干预措施: Coaching (Behavioral)

Faculty - control

No Intervention

Faculty members randomized to control arm. They are not paired with a trainee from this study and instead continue to provide mentorship as they typically would, as part of their role at an academic medical center

A controlled study examining CBT for bereaved children.Spuij, Reitz et al., 2012). Evidence that, in a small percentage of people, acute grief reactions turn into chronic debilitating distress, blocking reestablishment of normal routines, will likely lead to the inclusion of two bereavement-related disorders in the DSM-5, namely Adjustment Disorder Related to Bereavement, located in its main text, and Persistent Complex Bereavement-Related Disorder, located in its appendix (APA, 2000APA, 2012The death of a loved one in childhood and adolescence is a risk factor for distress and dysfunction. This event has been associated with increased emotional problems including elevated depression, anxiety, and posttraumatic stress, as well as somatic complaints and behavioural problems (Dowdney, 2008Kaplow, Layne, & Pynoos, 2012). An estimated 5% to 10% of children, experience clinically significant psychiatric problems following loss, including major depression, posttraumatic stress-disorder (PTSD), and Prolonged Grief Disorder (PGD) (Melhem, Moritz, Walker, Shear, & Brent, 2007Melhem, Porta, Shamseddeen, Walker, & Brent, 2011).PGD encompasses several symptoms including separation distress, preoccupation with thoughts about the lost person, a sense of purposelessness about the future, numbness, bitterness, difficulties accepting the loss and difficulty moving on with life without the lost person (Prigerson et al., 2009Shear et al., 2011). Although PGD has mostly been studied among adults, a growing body of empirical studies has shown that children and adolescents can develop PGD symptoms, that can be reliably assessed, are distinct from normal grief, depression and anxiety, including PTSD, and are predictive of significant concomitant internalizing and externalizing problems (Brown & Goodman, 2005Dillen, Fontaine, & Verhofstadt-Denève, 2008Spuij, Prinzie et al., 2012for discussions see Boelen & Prigerson, 2012Kaplow et al., 2012Wakefield, 2012).
